根因分析联合全方位护理对内镜下结直肠息肉切除患者心理状态、睡眠质量及康复结局的影响

Effects of root cause analysis combined with comprehensive nursing on psychological state,sleep quality and rehabilitation outcome of patients undergoing endoscopic colorectal polypectomy

目的探讨根因分析联合全方位护理对内镜下结直肠息肉切除患者心理状态、睡眠质量及康复结局的影响。方法选择110例内镜下结直肠息肉切除患者为研究对象,根据随机数字表法将其分为对照组和研究组,各55例。对照组接受常规护理,研究组在对照组的基础上开展根因分析联合全方位护理。比较两组的心理状态、术后康复相关指标、并发症发生情况、睡眠质量及生活质量改善情况。结果出院时,两组的中文版心理弹性量表(CD-RISC)各维度评分及总分均升高,且研究组高于对照组(P<0.05)。研究组的肠鸣音恢复时间、首次排气时间、首次进食时间、首次离床时间及住院时间短于对照组(P<0.05)。研究组的并发症总发生率低于对照组(P<0.05)。出院时,两组的匹兹堡睡眠指数量表(PSQI)评分均降低,胃肠道生活质量指数(GIQLI)评分均升高,且研究组优于对照组(P<0.05)。结论根因分析联合全方位护理可有效改善内镜下结直肠息肉切除患者的心理状态,减少术后并发症发生情况,缩短术后康复时间,改善睡眠质量,进而促进改善患者康复结局及生活质量。

Objective To explore the effects of root cause analysis combined with comprehensive nursing on psychological state,sleep quality and rehabilitation outcome of patients undergoing endoscopic colorectal polypectomy.Methods A total of 110 patients with endoscopic colorectal polypectomy were selected as the research objects and divided into control group and study group according to the random number table method,with 55 cases in each group.The control group received routine nursing,and the study group carried out root cause analysis combined with comprehensive nursing on the basis of the control group.The psychological status,postoperative rehabilitation related indicators,complications,sleep quality and quality of life improvement were compared between the two groups.Results At discharge,the scores of each dimension and total score of Connor-Davidson Resilience Scale(CD-RISC)in the two groups increased,and those in the study group were higher than the control group(P<0.05).The recovery time of bowel sounds,the first exhaust time,the first feeding time,the first time out of bed and the hospitalization time in the study group were shorter than those in the control group(P<0.05).The total incidence of complications in the study group was lower than that in the control group(P<0.05).At discharge,the Pittsburgh Sleep Quality Index(PSQI)score of the two groups decreased,and the Gastrointestinal Quality Life Index(GIQLI)score increased,and those in the study group were better than the control group(P<0.05).Conclusion Root cause analysis combined with comprehensive nursing can effectively improve the psychological state of patients undergoing endoscopic colorectal polypectomy,reduce postoperative complications,shorten postoperative rehabilitation time,improve sleep quality,and promote the improvement of rehabilitation outcome and quality of life of patients.
