实时超声弹性成像定量参数分析鉴别诊断子宫肌瘤与子宫腺肌症的价值与临床意义

摘要 目的:探讨实时超声弹性成像定量参数在鉴别诊断子宫肌瘤与子宫腺肌症的价值与意义。方法:选取2022年6月~2023年6月于本院就诊的疑似子宫肌瘤患者69例与疑似子宫腺肌症患者53例作为研究对象。所有患者均具备完整的临床资料,以手术病理结果作为诊断金标准分析实时超声弹性成像技术在鉴别诊断子宫肌瘤与子宫腺肌症中的作用,并分析弹性定量参数在鉴别诊断中的临床意义。结果:手术病理证实69例疑似子宫肌瘤患者中明确子宫肌瘤64例,53例疑似子宫腺肌症患者中明确诊断子宫腺肌症49例,以手术病理结果作为诊断金标准,术前实时超声弹性成像对子宫肌瘤诊断敏感性为93.75%,特异性为80.00%,准确性为92.75%,对子宫腺肌症诊断敏感性为93.88%,特异性为75.00%,准确性为92.45%;两组患者平均年龄、病灶平均直径比较无显著差异(P>0.05),但子宫肌瘤患者SR平均值明显高于子宫腺肌症,差异有统计学意义(P<0.05)。结论:实时超声弹性成像检查能够定量分析子宫肌瘤与子宫腺肌症的应变力比值,将其作为二者鉴别诊断依据具有极高的敏感性与特异性,并且实时超声弹性成像技术操作难度低、检查方便快捷,故可作为一种子宫肌瘤、子宫腺肌症无创鉴别诊断新技术,提高临床诊断与鉴别诊断能力。 Objective:To explore the value and significance of real-time ultrasound elastography quantitative parameters in the differential diagnosis of uterine fibroids and adenomyosis.Methods:69 patients with suspected uterine fibroids and 53 patients with suspected adenomyosis who visited our hospital from June 2022 to June 2023 were selected as the study subjects.All patients have complete clinical data,and the role of real-time ultrasound elastography technology in the differential diagnosis of uterine fibroids and adenomyosis is analyzed using surgical pathological results as the diagnostic gold standard.The clinical significance of elastic quantitative parameters in differential diagnosis is also analyzed.Results:Surgical pathology confirmed that out of 69 suspected patients with uterine fibroids,64 were diagnosed with uterine fibroids,and out of 53 suspected patients with adenomyosis,49 were diagnosed with adenomyosis.Using surgical pathology results as the diagnostic gold standard,preoperative real-time ultrasound elastography had a sensitivity of 93.75%,specificity of 80.00%,accuracy of 92.75%,and a sensitivity of 93.88%and specificity of 75.00%for the diagnosis of uterine adenomyosis,the accuracy was 92.45%;There was no significant difference in average age and lesion diameter between the two groups of patients(P>0.05),but the average SR value of patients with uterine fibroids was significantly higher than that of those with adenomyosis(P<0.05),and the difference was statistically significant.Conclusion:Real-time ultrasound elastography can quantitatively analyze the strain force ratio of uterine fibroids and adenomyosis,and it has high sensitivity and specificity as a basis for differential diagnosis between the two.In addition,real-time ultrasound elastography technology has low operational difficulty and convenient and fast examination.Therefore,it can be used as a new non-invasive differential diagnosis technology for uterine fibroids and adenomyosis,improving clinical diagnosis and differential diagnosis capabilities.
