雌激素在绝经后妇女萎缩性阴道炎治疗中的应用效果评价

Evaluation of the Application Effect of Estrogen in the Treatment of Atrophic Vaginitis in Postmenopausal Women

目的探讨雌激素在绝经后妇女萎缩性阴道炎治疗中的应用效果。方法将2022年3月至2023年5月萍乡矿业集团有限责任公司总医院收治的70例绝经后妇女萎缩性阴道炎患者进行研究。分组方法选择随机数字表法,分成两组各35例。对照组接受复方甲硝唑栓配合苦参凝胶治疗,观察组在对照组基础上加用雌激素治疗。比较两组患者疗程结束后的治疗效果、用药安全性,以及就诊时与疗程结束后阴道症状评分、阴道pH值、阴道细胞成熟指数(MV)、子宫内膜厚度、血清雌激素水平变化情况。结果疗程结束后,观察组总有效率(94.29%)高于对照组(77.14%),差异有统计学意义(P<0.05);观察组阴道症状评分、阴道pH值均低于对照组,MV高于对照组,差异有统计学意义(P<0.05);观察组雌二醇(E2)水平高于对照组,卵泡刺激素(FSH)水平低于对照组,差异有统计学意义(P<0.05)。两组治疗期间未见血尿常规、血生化指标、肝功能指标、血压、静息心率及体重的明显变化,也未见明显不良反应。结论雌激素配合复方甲硝唑栓与苦参凝胶能够提高绝经后妇女萎缩性阴道炎的治疗效果,改善患者的阴道环境,改善雌激素水平,且安全性良好。

Objective To explore the application effect of estrogen in the treatment of atrophic vaginitis in postmenopausal women.Methods A study was conducted on 70 postmenopausal women with atrophic vaginitis admitted to General Hospital of Pingxiang Mining Group Co.,Ltd from March 2022 to May 2023.The grouping method was chosen as the random number table method,with 35 cases in each groups.The control group was treated with compound metronidazole suppositories combined with sophora flavescens gel,and the observation group was treated with estrogen on the basis of the control group.Compare the treatment efficacy,medication safety,and changes in vaginal symptom scores,vaginal pH,vaginal cell maturation index(MV),endometrial thickness,and serum estrogen levels between two groups after the end of treatment.Results After the end of the treatment,the total effective rate of the observation group was 94.29%,which was higher than the control group's 77.14%,with a significant difference in statistical results(P<0.05);the vaginal symptom score and vaginal pH value in the observation group were lower than those in the control group,and the MV was higher than that in the control group,with significant differences(P<0.05);the level of estradiol(E2)in the observation group was higher than that in the control group,while the level of follicle stimulating hormone(FSH)was lower than that in the control group,with significant differences(P<0.05).There were no significant changes in blood and urine routine,blood biochemical indicators,liver function indicators,blood pressure,resting heart rate,and body weight,and no significant adverse reactions were observed.Conclusion Estrogen combined with compound metronidazole suppositories and sophora flavescens gel can improve the therapeutic effect of atrophic vaginitis in postmenopausal women,improve the vaginal environment of patients,improve the estrogen level,and have good safety.
