颈动脉超声检查在脑卒中高危人群筛查中的应用价值

Application Value of Carotid Artery Ultrasound in the Screening of High Risk Population of Stroke

目的:探讨颈动脉超声检查在脑卒中高危人群筛查中的应用价值。方法:选取2022年1月—2023年12月于北京市大兴区榆垡镇中心卫生院进行脑卒中筛查的200例受检者作为研究对象,均接受颈动脉超声检查,以数字减影血管造影确诊结果为“金标准”,分析颈动脉超声的检出率及诊断效能。结果:200例脑卒中高危受检者经数字减影血管造影确诊118例,经颈动脉超声诊断为脑卒中121例。颈动脉超声诊断脑卒中的准确度为92.50%(185/200),特异度为89.02%(73/82),灵敏度为94.92%(112/118),阳性预测值为92.56%(112/121),阴性预测值为92.41%(73/79)。颈动脉超声与数字减影血管造影诊断结果的一致性较好(Kappa=0.805,P<0.001)。结论:颈动脉超声检查在脑卒中高危人群筛查中的应用效果显著,诊断准确率较高,与数字减影血管造影诊断结果的一致性较好,能够为临床诊断工作的开展提供有效的影像学依据。

Objective:To investigate the application value of carotid artery ultrasound in the screening of high risk population of stroke.Methods:A total of two hundred patients undergoing stroke screening in Beijing Daxing District Yufa Town Central Health Center from January 2022 to December 2023 were selected as the study objects,and all of them received carotid artery ultrasound examination.Using the digital subtraction angiography as the"gold standard",the detection rate and diagnostic efficiency of carotid artery ultrasound were analyzed.Results:Among 200 patients at high risk of stroke,118 cases were diagnosed by digital subtraction angiography and 121 cases were diagnosed by carotid ultrasound;the accuracy of carotid ultrasound diagnosis of stroke was 92.50%(185/200),the specificity was 89.02%(73/82),the sensitivity was 94.92%(112/118),the positive predictive value was 92.56%(112/121)and the negative predictive value was 92.41%(73/79);there was good agreement between carotid ultrasound and digital subtraction angiography(Kappa=0.805,P<0.001).Conclusion:The application of carotid artery ultrasound in the screening of high-risk groups of stroke has significant effect,high diagnostic accuracy,and good consistency with the diagnostic results of digital subtraction angiography,which can provide effective imaging basis for clinical diagnosis.
