ERAS理念下卵巢、输卵管疾病患者腹腔镜手术并发症及其影响因素分析

Analysis on Laparoscopic Surgery Complications in Patients with Ovarian and Fallopian Tube Disease Under ERAS Concept and Their Influencing Factors

目的:分析ERAS理念下卵巢、输卵管疾病患者腹腔镜手术并发症及其影响因素。方法:选取2022年9月—2023年9月于乌鲁木齐市妇幼保健院行腹腔镜手术的卵巢、输卵管的良性病变患者100例作为研究对象,所有患者均进行ERAS理念干预,记录患者的一般资料,分析ERAS理念下行腹腔镜手术的卵巢、输卵管的良性病变患者并发症发生的影响因素。结果:100例行腹腔镜手术的卵巢、输卵管的良性病变患者中出现并发症23例(23%),未出现并发症77例(77%)。腹部手术史、术中气腹压、子宫内膜异位症病史、医生工作年限、术后卧床时间、导尿拔除时间是ERAS理念下卵巢、输卵管疾病患者腹腔镜手术并发症发生的影响因素(P<0.05)。腹部手术史、术中气腹压≥15 mmHg、子宫内膜异位症病史、术后卧床时间≥5 d、导尿管拔除时间术后6 h是ERAS理念下卵巢、输卵管疾病患者腹腔镜手术并发症发生的危险因素(P<0.05),医生工作年限>5年是ERAS理念下卵巢、输卵管疾病患者腹腔镜手术并发症发生的保护因素(P<0.05)。结论:ERAS理念下卵巢、输卵管疾病患者腹腔镜手术后并发症与多种因素有关,临床应采取针对性的应对措施进行护理,降低并发症的发生风险,促进患者康复。

Objective:To analyze the laparoscopic surgery complications in patients with ovarian and fallopian tube disease under ERAS concept and their influencing factors.Methods:A total of 100 patients with benign ovarian and fallopian tube lesions undergoing laparoscopic surgery in Urumqi Maternal and Child Health Hospital from September 2022 to September 2023 were selected as the study subjects.All patients underwent ERAS concept intervention.General data of the patients were recorded.The influencing factors of complications in patients with benign ovarian and fallopian tube lesions undergoing laparoscopic surgery under ERAS concept were analyzed.Results:Among the 100 patients with benign ovarian and fallopian tube lesions undergoing laparoscopic surgery,23 cases(23%) had complications and 77 cases(77%) had no complications.History of abdominal surgery,intraoperative pneumoperitoneum pressure,history of endometriosis,working years of doctors,and postoperative bed time were the influencing factors for the occurrence of laparoscopic surgery complications in patients with ovarian and fallopian tube diseases under the ERAS concept(P<0.05).History of abdominal surgery,intraoperative pneumoperitoneum pressure ≥ 15 mmHg,history of endometriosis,postoperative bed time ≥ 5 d,and catheter removal time of 6 h postoperatively swere risk factors for the occurrence of laparoscopic surgery complications in patients with ovarian and fallopian tube diseases under the ERAS concept(P<0.05).The working years of doctors > 5 years was protective factor for the occurrence of laparoscopic surgery complications in patients with ovarian and fallopian tube diseases under the ERAS concept(P<0.05).Conclusion:The complications after laparoscopic surgery in patients with ovarian and fallopian tube diseases under the ERAS concept are related to many factors.In clinical setting,it is necessary to take targeted countermeasures to reduce the risk of complications and promote the recovery of patients.
