老年胃癌患者根治术后恐惧疾病进展水平及其影响因素分析

Analysis of level of fear of disease progression and its influencing factors in elderly gastric cancer patients after radical surgery

目的调查老年胃癌根治术患者术后恐惧疾病进展水平并分析其影响因素。方法选取2019年4月—2023年4月在空军军医大学空军第九八六医院诊治的80例老年胃癌根治术患者,根据患者术后是否发生恐惧疾病进展将其分为术后恐惧疾病进展组(n=53)和术后无恐惧疾病进展组(n=27)。对2组一般资料比较并进行单因素分析,采用Logistic回归分析影响术后恐惧疾病进展的危险因素。结果本研究80例老年胃癌根治术患者中,术后恐惧进展疾病发生率为66.25%。2组子女情况、家庭人均月收入、术前访视情况、辅助放化疗情况和家人陪伴时间比较,差异均有统计学意义(P<0.05)。Logistic回归分析结果显示,患者未接受术前访视和家人陪伴时间≤5次/周为影响老年胃癌根治术患者术后恐惧疾病进展的独立危险因素(P<0.05)。结论老年胃癌根治术患者术后恐惧疾病进展发生率较高,临床上可通过强化术前访视和增加家人陪伴,以降低患者术后恐惧疾病进展水平。

Objective To investigate the level of fear of disease progression and analyze its influencing factors in elderly patients after radical gastrectomy.Methods A total of 80 elderly patients undergoing radical gastrectomy in Hospital 986 of Air Force,Air Force Military Medical University from April 2019 to April 2023 were selected.According to presence or absence of postoperative fear of disease progression,they were divided into fear group(n=53)and non-fear group(n=27).The general data of the two groups were analyzed by univariate analysis.Logistic regression was used to analyze the risk factors affecting the postoperative fear of disease progression.Results In this study of 80 elderly patients,the incidence of postoperative fear of disease progression was 66.25%.There were significant differences in filial situations,monthly family income per capita,preoperative visit,adjuvant chemoradiotherapy and family accompanying time between the two groups(P<0.05).The results of logistic regression analysis showed that no preoperative visit and family accompanying time≤5 times/week were independent risk factor for fear of disease progression in elderly patients after radical gastrectomy(P<0.05).Conclusion The incidence of fear of disease progression is higher in elderly patients after radical gastrectomy.In clinical practice,strengthening preoperative visits and increasing family accompanying can reduce postoperative fear of disease progression.
