小儿肺炎支原体感染合并消化系统损害的现状调查及高危因素分析

摘要 [目的]探讨小儿肺炎支原体感染并发消化系统损害的临床特征及危险因素分析,以期给临床诊治提供参考。[方法]选择肺炎支原体感染患儿200例,详细记录患儿基本临床资料,采用多因素Logistic回归分析影响肺炎支原体感染合并消化系统损害的独立危险因素。[结果]200例肺炎支原体感染患儿中,消化系统损伤者(研究组)64例,发生率为32.0%;无消化系统损伤者(对照组)136例。经单因素分析显示,研究组与对照组发热持续时间、发热程度、抗生素应用时间、C反应蛋白、红细胞沉降率水平上比较差异有统计学意义(P<0.05)。经多因素Logistic回归分析显示,发热持续时间≥7 d、发热程度为高热人数占比、抗生素应用时间为影响小儿肺炎支原体感染合并消化系统损害的独立危险因素。[结论]小儿肺炎支原体感染合并消化系统损害发生率较高,且受发热持续时间≥7 d、发热程度为高热人数占比、抗生素应用时间等多因素影响,临床上应重点关注此类患儿。 [Objective]To explore the clinical characteristics and risk factors of digestive system damage caused by Mycoplasma pneumoniae infection in children,so as to provide reference for clinical diagnosis and treatment.[Methods]The totals of 200 children with Mycoplasma pneumoniae infection were divided into the study group(64 cases)and the control group(136 cases)according to whether they were combined with digestive system injury.The basic clinical data of the children were recorded in detail,and the independent risk factors of Mycoplasma pneumoniae infection complicated with digestive system damage were analyzed by multivariate logistic regression.[Results]Among 200 children with Mycoplasma pneumoniae infection,64 cases had digestive system injury,and the incidence rate was 32.0%.Univariate analysis showed that there were significant differences between the two groups in fever duration,fever degree,antibiotic application time,CRP and erythrocyte sedimentation rate(P<0.05).Multivariate logistic regression analysis showed that the duration of fever≥7 d,the degree of fever was the proportion of high fever,and the application time of antibiotics were independent risk factors for Mycoplasma pneumoniae infection in children with digestive system damage.[Conclusion]The incidence of Mycoplasma pneumoniae infection combined with digestive system damage in children is high,and it is affected by multi factors,such as the duration of fever≥7 d,the proportion of people with high fever,and the application time of antibiotics.Clinical attention should be paid to such children.
