终末期肾衰竭血透患者心理痛苦现状及其相关影响因素分析

Analysis of psychological distress and related factors in end-stage renal failure hemodialysis patients

目的:探讨终末期肾衰竭血透患者心理痛苦的影响因素。方法:选取医院接收的81例终末期肾衰竭血透患者,采用心理痛苦温度计(DT)评估心理痛苦现状,经线性回归分析其影响因素。结果:81例患者DT评分为(4.35±1.04)分,处于中度痛苦水平;病耻感(SIS)评分为(41.36±5.19)分。不同文化程度、透析时长、希望水平资料患者DT评分比较,差异有统计学意义(P<0.05)。经Pearson检验,病耻感与心理痛苦呈正相关(r=5.106,P<0.05);线性回归分析结果显示,文化程度为初中及以下、透析时长≥1年、希望水平低、病耻感重均为心理痛苦的影响因素(P<0.05)。结论:终末期肾衰竭血透患者心理痛苦度受文化程度、透析时长、希望水平及病耻感等因素影响。

Objective:To analyze the influencing factors of psychological distress in end-stage renal failure hemodialysis patients.Method:81 end-stage renal failure hemodialysis patients admitted to hospital were selected.The psychological distress status was evaluated using a psychological distress thermometer(DT),and the influencing factors were analyzed through linear regression.The DT score of 81 patients was(4.35±1.04),indicating moderate pain level;The Social Impact Scale(SIS)score is(41.36±5.19).Comparison of DT scores among patients with different educational levels,dialysis duration,and hope level data(P<0.05).According to Pearson's test,there is a positive correlation between shame and psychological pain(r=5.106,P<0.05);linear regression analysis results show that education level of junior high school or below,dialysis time≥1 year,low hope level,and heavy shame are all influencing factors of psychological pain(P<0.05).Conclusion:The psychological distress of end-stage renal failure hemodialysis patients is influenced by factors such as educational level,dialysis duration,hope level,and disease shame.
