摘要
目的探讨行同步放射治疗和化学治疗(简称“放化疗”)的宫颈癌患者应用目标导向式健康教育结合阶段性护理策略的效果。方法以2019年9月至2022年9月在华中科技大学同济医学院附属同济医院行同步放化疗的110例宫颈癌患者为研究对象,按随机数字表法分为对照组和观察组,各55例。对照组给予常规护理,观察组实施目标导向式健康教育结合阶段性护理策略。比较2组的焦虑自评量表(SAS)、抑郁自评量表(SDS)、癌症自我管理效能感量表(SUPPH)和生命质量测定量表(QLQ-C30)得分,以及血清糖链抗原125(CA125)、癌胚抗原(CEA)和鳞状上皮癌相关抗原(SCCA)的水平,同时比较2组的不良反应发生率。结果干预6周和12周后,观察组的SAS和SDS得分,以及CA125、CEA和SCCA水平均低于同期对照组(P均<0.05),且2组均低于干预前(P均<0.05)。干预6周和12周后,观察组的SUPPH和QLQ-C30得分均高于同期对照组(P均<0.05),且2组均比干预前高(P均<0.05)。干预12周后,观察组不良反应的总发生率比对照组低(P均<0.05)。结论目标导向式健康教育结合阶段性护理策略可减轻行同步放化疗的宫颈癌患者的不良情绪,提高其自我管理效能和生活质量,降低血清肿瘤标志物水平和不良反应发生率。
Objective To explore the effect of goal-oriented health education combined with phased nursing strategies in cervical cancer patients undergoing synchronous radiotherapy and chemotherapy(referred to as radiochemotherapy).Methods A total of 110 cervical cancer patients who underwent concomitant radiochemotherapy at Tongji Hospital,Tongji Medical College of Huazhong University of Science and Technology from September 2019 to September 2022 were selected as the study subjects.They were randomly divided into a control group and an observation group using a random number table method,with 55 cases in each group.The control group received routine care,while the observation group implemented goal-oriented health education combined with phased nursing strategies.The scores of the Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Strategies Used by Patients to Promote Health(SUPPH),and Quality of Life Questionnaire-Core 30(QLQ-C30)were compared between the two groups,as well as the levels of carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA),and squamous cell carcinoma antigen(SCCA).The incidence of adverse reactions was compared between the two groups.Results After 6 and 12 weeks of intervention,the SAS and SDS scores,as well as CA125,CEA,and SCCA levels of the observation group were lower than those of the control group during the same period(all P<0.05),and the above scores and indicators of both groups were lower than those before intervention(all P<0.05).After 6 and 12 weeks of intervention,the SUPPH and QLQ-C30 scores of the observation group were higher than those of the control group during the same period(all P<0.05),and the above scores of both groups were higher than those before intervention(all P<0.05).After 12 weeks of intervention,the total incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Goal-oriented health education combined with phased nursing strategies can alleviate the negative emotions of cervical cancer patients undergoing concomitant radiochemotherapy,improve their self-management efficiency and quality of life,and reduce serum tumor marker levels and incidence of adverse reactions.
