基于运动想象的脑机接口训练系统对脑卒中患者上肢功能的影响

摘要 目的分析基于运动想象的脑机接口训练系统对脑卒中患者上肢功能的影响。方法选取脑卒中患者56例,按随机数字表法分为试验组和对照组,脱落3例,最终入组53例,试验组26例,对照组27例。对照组在常规康复基础上增加康复机械手训练,试验组在常规康复基础上增加基于运动想象的脑机接口训练系统,外周设备匹配机械手。在干预前和治疗4周后采用Fugl-Meyer上肢部分量表(FMA-UE)评估上肢运动功能,采用改良Barthel指数(MBI)评估日常生活活动能力(ADL),采用加拿大作业表现量表(COPM)评估作业表现和治疗满意度。结果FMA-UE评分对照组和试验组治疗后均较治疗前有明显改善,差异有统计学意义(P<0.01),且试验组治疗后改善较对照组更为明显,差异有统计学意义(P<0.05)。MBI评分对照组和试验组治疗后均较治疗前有明显改善,差异有统计学意义(P<0.01),且试验组治疗后改善较对照组更为明显,差异有统计学意义(P<0.05)。COPM评分中表现总分差值与满意总分差值试验组均明显高于对照组,差异有统计学意义(P<0.01)。结论MI-BCI训练系统有助于改善脑卒中后上肢运动功能、日常生活活动能力和作业表现能力,并提高治疗满意度。 Objective To analyze the influence of brain-computer interface training system based on motor imagination on upper limb function of stroke patients.Method 56 cases of stroke patients were ran-domly divided into experimental group and control group,with 3 cases falling off and 53 cases finally en-tering the group,26 cases in the experimental group and 27 cases in the control group.The control group added rehabilitation manipulator training on the basis of routine rehabilitation,while the experimental group added brain-computer interface training system based on motor imagination on the basis of routine rehabilitation,and the peripheral equipment matched the manipulator.Fugl-Meyer Assessment of Upper Extremity(FMA-UE)was used to evaluate upper limb motor function before intervention and after treat-ment for 4 weeks,and modified Barthel index(modified Barthel index,MBI)was used to evaluate activi-ties of daily living(ADL),and Canadian Occupative Performance Measure(COPM)was used to evaluate job performance and treatment satisfaction.Results The scores of FMA-UE in the control group and the experimental group were significantly improved after treatment(P<0.01),and the improvement in the ex-perimental group was more obvious than that in the control group(P<0.05).The MBI scores of the control group and the experimental group were significantly improved after treatment,the difference was statisti-cally significant(P<0.01),and the improvement of the experimental group was more obvious than that of the control group,the difference was statistically significant(P<0.05).The difference between the perfor-mance total score and the satisfaction total score in the COPM score was significantly higher in the experi-mental group than that in the control group,with statistical significance(P<0.01).Conclusion MI-BCI training system is helpful to improve upper limb motor function,activity of daily living and performance of work after stroke,and improve treatment satisfaction.
