基于超声定位针刀治疗腕管综合征的临床疗效观察

Clinical Efficacy Observation of Ultrasound-Based Localization of Needle Knife in the Treatment of Carpal Tunnel Syndrome

目的评价基于超声定位针刀治疗腕管综合征(carpal tunnel syndrome,CTS)的临床疗效。方法选择2021年9月至2023年8月符合入选标准的腕管综合征患者80例,分为对照组和治疗组,各40例,对照组采用针刀治疗,治疗组在对照组基础上配合超声定位,每周1次,连续治疗4周。根据视觉模拟评分法(VAS)、波士顿腕管量表(BCTQ)、钩骨钩腕横韧带厚度(THTCL),评估其临床疗效。结果治疗后,两组患者VAS评分、总体BCTQ评分均低于治疗前,且治疗组VAS评分、总体BCTQ评分优于对照组(P<0.05)。治疗后,两组患者THTCL低于治疗前,但两组治疗后厚度对比无统计学意义(P>0.05)。治疗组总有效率高于对照组(P<0.05)。结论针刀治疗腕管综合征疗效确切,而在超声指导下的针刀疗法更具优越性。

Objective To evaluate the clinical efficacy of ultrasound-guided acupotomy in the treatment of carpal tunnel syndrome(CTS).Methods A total of 80 patients with carpal tunnel syndrome who met the inclusion criteria from September 2021 to August 2023 were selected and divided into a control group and a treatment group,each with 40 cases;the control group was treated with needle knife,and the treatment group was treated with ultrasound localization on the basis of the control group once a week for 4 consecutive weeks.The clinical efficacy was evaluated according to the visual analog scale(VAS),Boston Carpal Tunnel Scale(BCTQ),and thickness of hooked carpal transverse ligament of the hook bone(THTCL).Results After treatment,the VAS scores and overall BCTQ scores of patients in both groups were lower than before treatment,and the VAS scores and overall BCTQ scores of the treatment group were better than those of the control group(P<0.05).After treatment,the THTCL of patients in both groups was lower than that before treatment,but the comparison of post-treatment thickness between the two groups was not statistically significant(P>0.05).The overall effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusions Acupuncture and knife therapy for carpal tunnel syndrome is efficacious,and ultrasound-guided acupuncture and knife therapy is even more superior.
