基于加速康复外科理念的营养干预对胸腔镜肺癌切除术患者的效果

Effectiveness of Nutritional Intervention Based on Accelerated Rehabilitation Surgical Concept in Patients Undergoing Thoracoscopic Lung Cancer Resection

摘要 目的探讨基于加速康复外科(ERAS)理念的营养干预对胸腔镜肺癌切除术患者的影响。方法选取本院2021年1月至2022年12月收治的92例肺癌切除术患者,按入院顺序随机分为两组,各46例,对照组给予常规营养干预,研究组予以ERAS理念营养干预,对比两组营养状况、术后恢复情况和并发症发生情况。结果两组干预后BMI、血清白蛋白、前白蛋白、转铁蛋白、三角肌皮褶厚度水平均有所降低,但研究组水平较对照组更高(P<0.05)。研究组首次下床时间、日下床活动时间、拔管时间、住院时间均短于对照组,术后1~3 d的胸腔引流量低于对照组(P<0.05)。研究组肺不张、肺部感染、心律失常、尿潴留等并发症发生率为4.35%,较对照组的19.57%低(P<0.05)。结论对胸腔镜肺癌切除术患者实施基于ERAS理念的营养干预能有效改善患者的营养状况,促进患者术后快速恢复,降低并发症的发生。 Objective To explore the effects of nutritional intervention based on the concept of enhanced recovery after surgery(ERAS)on patients undergoing thoracoscopic lung cancer resection.Methods A total of 92 lung cancer resection patients admitted from January 2021 to December 2022 were selected and randomly divided into two groups of 46 cases each according to the order of admission;the control group was given conventional nutritional intervention,and the research group was given nutritional intervention based on the ERAS concept to compare the nutritional status,postoperative recovery,and the occurrence of complications between the two groups.Results The levels of BMI,serum albumin,prealbumin,transferrin,and deltoid skinfold thickness were reduced in both groups after the intervention,but the levels were higher in the study group than in the control group(P<0.05).The study group's first time out of bed,daily time out of bed activity,time to extubation,and hospital stay were shorter than the control group,and the chest drainage was lower than that of the control group in the first to third postoperative days(P<0.05).The complication rate of pulmonary atelectasis,pulmonary infection,cardiac arrhythmia,and urinary retention in the study group was 4.35%,which was lower than that of 19.57%in the control group(P<0.05).Conclusions The implementation of nutritional intervention based on the ERAS concept for patients undergoing thoracoscopic lung cancer resection can effectively improve the nutritional status of patients,promote the rapid recovery of patients after surgery,and reduce the occurrence of complications.
