肌红蛋白联合肌钙蛋白检测在早期急性心肌梗死临床诊断中的应用分析

Analysis of the Clinical Application of Combined Myoglobin and Troponin Testing in the Early Diagnosis of Acute Myocardial Infarction

目的探讨肌红蛋白(MB)与肌钙蛋白I(CTnI)联合检测应用下对急性心肌梗死的诊断效果。方法回顾性选取剑河县人民医院2023年1月至2023年12月收治的90例急性心肌梗死患者(观察组),并选取同期于剑河县人民医院进行体检的健康者50例(对照组),比较两组MB、cTnI水平,并分析MB、CTnI与两者联合在急性心肌梗死诊断中的价值。结果观察组MB与CTnI水平均显著高于对照组(P<0.001)。MB与CTnI联合检测诊断急性心肌梗死的准确度、特异度、灵敏度分别为95.00%、93.55%、96.55%,均高于MB、CTnI单独检测的诊断效能,且CTnI联合MB和冠状动脉造影的一致性最高,Kappa系数为0.9。结论在对急性心肌梗死患者的诊断中,与单一MB、CTnI指标检测相比,两者联合的诊断效能有更高的价值和意义,具有较高的诊断价值。

Objective To explore the diagnostic efficacy of combined detection of myoglobin(MB)and cardiac troponin I(cTnI)in patients with acute myocardial infarction(AMI).Methods Retrospectively select 90 patients with acute myocardial infarction(observation group)admitted to the People's Hospital of Jianhe County from January 2023 to December 2023,and select 50 healthy people who underwent physical examination at the same time in the People's Hospital of Jianhe County(control group).Compare the MB and cTnI levels of the two groups,and analyze the diagnostic value of MB,CTnI,and their combination in acute myocardial infarction.Results The levels of MB and cTnI in the observation group were significantly higher than those in the control group(P<0.001).The accuracy,specificity,and sensitivity of the combined detection of MB and cTnI in diagnosing acute myocardial infarction were 95.00%,93.55%,and 96.55%,respectively,which were higher than the diagnostic efficiency of MB and cTnI alone.Moreover,the consistency between cTnI joint MB and coronary angiography was the highest,with a Kappa coefficient of 0.9.Conclusion Compared with the single MB and cTnI index detection,the combined diagnosis of both has higher value and significance in the diagnosis of patients with acute myocardial infarction,and it has a higher diagnostic value.
