心理护理联合损伤控制在骨科下肢创伤临床急救中的应用研究

Study on the Application of Psychological Care Combined with Damage Control in the Emergency Treatment of Lower Limb Trauma in Orthopedics

目的分析在常规急救手段的基础上,采取心理护理联合损伤控制在下肢创伤患者中的应用效果。方法选取2021年5至2023年5月本院收治的84例下肢创伤患者,按照随机数字分配法分为观察组和对照组,每组各42例,观察组给予心理护理和损伤控制,对照组采取传统治疗手段。比较两组患者心理状态评分、依从性、满意度、膝关节功能、踝关节功能、术后并发症。结果干预后,观察组SAS、SDS评分均显著低于对照组(P<0.05);观察组依从性评分显著高于对照组(P<0.05)。手术前,两组患者膝关节临床、运动功能评分,踝关节功能、疼痛、力线评分均无显著差异(P>0.05);术后3个月,观察组膝关节临床、运动功能评分,踝关节功能、疼痛、力线评分显著高于对照组(P<0.05)。观察组术后并发症发生率为2.38%,显著低于对照组的19.05%(P<0.05)。结论心理护理联合损伤控制可有效改善下肢创伤患者焦虑、抑郁不良情绪,提高患者治疗依从性,有利于患者下肢功能恢复,降低术后并发症。

Objective To analyze the effectiveness of combining psychological care with damage control on lower limb trauma patients in addition to conventional emergency measures.Methods 84 patients with lower limb trauma admitted to our hospital from May 2021 to May 2023 were randomly divided into theobservation group and the control group,with 42 cases in each group.The observation group received psychological care and injury control,while the control group received traditional treatment.We compared the psychological status scores,compliance,satisfaction,knee joint function,ankle joint function,and postoperative complications between two groups of patients.Results After the intervention,the observation group exhibited significantly lower SAS and SDS scores compared to the control group(P<0.05).Additionally,the compliance scores of the observation group were significantly higher than those of the control group(P<0.05).Before surgery,there were no significant differences between the two groups in terms of clinical and functional scores of the knee joint,ankle joint function,pain,and alignment scores(P>0.05).Three months postoperatively,the observation group demonstrated significantly higher clinical and functional scores for the knee joint,as well as improved ankle joint function,pain relief,and alignment scores compared to the control group(P<0.05).Additionally,the incidence of postoperative complications was significantly lower in the observation group(2.38%)compared to the control group(19.05%)(P<0.05).Conclusion Combining psychological care with damage control can effectively alleviate anxiety and depression in lower limb trauma patients,enhance patient compliance,promote the recovery of lower limb function,and reduce postoperative complications.
