头痛之病机核心在于“湿浊”,且与肝气郁滞相关联。湿浊之邪存内,阻滞气血运行,则清阳之气、濡养之血不能上荣头目清窍,脉络空虚,头痛乃发。肝主疏泄,调畅气机,肝郁不舒则气机不畅,疏泄失职,清空受扰,发为头痛。临证从“湿浊”论治,化裁经典名方散偏汤,重用土茯苓利湿泻浊通络,川芎祛风疏肝止痛,活用引经药,不忘虫类药搜剔驱邪,强调顾护脾胃,重视失眠焦虑状况,并适当兼顾鼻鼽、风眩诸症,有效缓解头痛。

The core pathogenesis of headache lies in"Dampness",which is associated with Liver Qi stagnation.When the evil factors of Dampness and Turbidity exist in body and block the flow of Qi and blood,the clear Yang Qi and the Yin blood cannot nourish the head and eyes,which induces the emptiness of the meridian and collateral,and the headache takes place.The Liver governs the conveyance and dispersion and regulates Qi movement.If the Liver Qi is depressed the Qi movement is not smooth.With conveyance and dispersion dysfunction,the clear head is disturbed,and the headache occurs.In clinical practice,headache can be treated from perspective of"Dampness and Turbidity",Modified classic prescription Sanpian Decoction can be used,in which Tufuling(Poria cocos)functions in relieving Dampness and Turbidity and dredging colleteral,Chuanxiong functions in dispelling Wind,soothing the Liver Qi and stopping pain.In addition,meridian inducing drugs can be used flexibly,and insect drugs can be used to drive away evil factors.Besides,the care of the Spleen and Stomach should be emphasized.When attaching importance to insomnia and anxiety,the symptoms of nasal snorting and Wind dizziness should also be considered appropriately,to effectively relieve headache.
