健康体检人群骨代谢指标的检测分析

摘要 目的:本文旨在分析探讨北京地区13~92岁不同年龄段健康体检者骨代谢4项指标的分布情况及健康管理建议。方法:选择2022年1月至2023年12月健康受检者2929例作为研究对象,检测25羟基维生素D[25(OH)D]、骨钙素N端中分子片段(N-MID)、总Ⅰ型胶原蛋白氨基端延长肽(P1NP)、β-胶原特殊系列(β-CTX)4项骨代谢指标。按照年龄分为≤19岁、20~29岁、30~39岁、40~49岁、50~59岁、60~69岁、≥70岁,对比不同年龄段、不同性别骨代谢4项指标水平,且分别对比男性及女性不同年龄段骨代谢水平。结果:不同年龄段25(OH)D、N-MID、P1NP、β-CTX骨代谢4项指标对比差异具有统计学意义(F=51.463、299.392、441.541、162.583,P<0.05)。25(OH)D随着年龄增长而升高,N-MID、P1NP、β-CTX随着年龄增长而降低。即使25(OH)D随着年龄增长而升高,但是普遍不足。不同性别体检者25(OH)D、P1NP、β-CTX骨代谢3项指标对比差异有统计学意义(P<0.05),N-MID对比差异无统计学意义(P>0.05),其中,男性的25(OH)D高于女性(P<0.05)。男性体检者中,不同年龄段25(OH)D、N-MID、P1NP、β-CTX骨代谢4项指标对比差异具有统计学意义(F=29.433、304.862、409.65、195.333,P<0.05)。女性体检者中,不同年龄段25(OH)D、N-MID、P1NP、β-CTX骨代谢4项指标对比差异具有统计学意义(F=23.382、102.413、169.254、70.833,P<0.05)。结论:骨代谢指标与年龄相关性强,检测骨代谢水平可以为体检人群健康管理提供针对性的个体化建议。 Objective:The purpose of this study was to analyze the distribution of 4 indexes of bone metabolism in healthy people aged 13 to 92 years in Beijing area and the suggestions on health management.Methods:From January 2022 to December 2023,2929 cases of physical examination were selected as the study objects,and 4 bone metabolic indexes of 25 hydroxyvitamin D[25(OH)D],N-terminal molecular fragment of osteocalcin(N-MID),amino terminal prolongating peptide of total type I collagen(P1NP)andβ-collagen special series(β-CTX)were detected.The four bone metabolism markers were measured.The participants were divided into age groups:≤19 years,20-29 years,30-39 years,40-49 years,50-59 years,60-69 years,and≥70 years.The bone metabolism markers were compared among different age groups and between genders.Results:There were significant differences in the four bone metabolism markers(25(OH)D,N-MID,P1NP,andβ-CTX)among different age groups(F=51.463,299.392,441.541,162.583;P<0.05).25(OH)D levels increased with age,while N-MID,P1NP,andβ-CTX levels decreased with age.However,25(OH)D levels were generally insufficient even with increasing age.There were significant differences in 25(OH)D,P1NP,andβ-CTX between genders(P<0.05),but not in N-MID(P>0.05).Specifically,25(OH)D levels were higher in males compared to females(P<0.05).Among male participants,there were significant differences in the four bone metabolism markers among different age groups(F=29.433,304.862,409.65,195.333;P<0.05).Among female participants,there were significant differences in the four bone metabolism markers among different age groups(F=23.382,102.413,169.254,70.833;P<0.05).Conclusion:Bone metabolism may be high related to age.These findings provide targeted and individualized recommendations for health management.
