益肾化痰祛瘀方治疗绝经后骨质疏松症(肾虚血瘀型)的效果及对氧化应激的影响

Effect of Kidney-Replenishing,Phlegm-Resolving and Stasis-Dispelling Formula on Postmenopausal Osteoporosis of Kidney Deficiency and Blood Stasis Type and on Oxidative Stress

目的:观察益肾化痰祛瘀方治疗绝经后骨质疏松症(肾虚血瘀型)的效果及对氧化应激的影响。方法:选择2020年1月—2022年12月于启东市中医院就诊的肾虚血瘀型绝经后骨质疏松症患者102例,采用随机数字表法分成对照组与观察组,每组51例。对照组给予阿仑膦酸钠片与碳酸钙D3咀嚼片口服治疗,观察组在此基础上给予益肾化痰祛瘀方口服治疗。观察两组临床疗效、中医证候积分、骨密度(BMD)、氧化应激产物水平与不良反应。结果:观察组患者治疗总有效率高于对照组(P<0.05);治疗后,两组患者中医证候积分、高级氧化蛋白产物(AOPP)与血浆蛋白羰基(PCO)水平均降低(P<0.05),观察组低于对照组(P<0.05);治疗后,两组患者超氧化物歧化酶(SOD)、总抗氧化能力(T-AOC)水平均升高(P<0.05),观察组高于对照组(P<0.05);两组不良反应总发生率差异无统计学意义(P>0.05)。结论:益肾化痰祛瘀方治疗肾虚血瘀型绝经后骨质疏松症可有效改善临床症状,增加骨密度,减轻氧化应激反应,且安全性较高。

Objective:To observe the effect of kidney-replenishing,phlegm-resolving and stasis-dispelling formula on postmenopausal osteoporosis of kidney deficiency and blood stasis type and on oxidative stress.Methods:A hundred and two postmenopausal osteoporosis patients with kidney deficiency and blood stasis who were treated in Qidong Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected and divided into conventional group and traditional Chinese medicine group by random number table method,with 51 patients in each group.The conventional group was treated with alendronate sodium tablets and calcium carbonate D3 chewable tablets,and the traditional Chinese medicine group was treated the same plus kidney-replenishing,phlegm-resolving and stasis-dispelling formula orally taken.The clinical efficacy,TCM syndrome score,bone mineral density(BMD),levels of oxidative stress products and adverse reactions were observed in the 2 groups.Results:The total effective rate of the traditional Chinese medicine group was higher than that of conventional group(P<0.05).After treatment,TCM syndrome score,advanced oxidized protein product(AOPP)and plasma protein carbonyl(PCO)levels in the 2 groups decreased,and they were lower in traditional Chinese medicine group than in the conventional group(P<0.05).The levels of superoxide dismutase(SOD)and total antioxidant capacity(T-AOC)in the 2 groups increased,and the levels in TCM group were higher than those in conventional group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The treatment of postmenopausal osteoporosis with kidney-replenishing,phlegm-resolving and stasis-dispelling formula can effectively improve clinical symptoms,increase bone mineral density,reduce oxidative stress,and has high safety.
