彩色多普勒超声在视网膜脱离患者中的诊断效能及其与二维超声的比较研究

The diagnostic efficacy of Color Doppler Ultrasonography in patients with retinal detachment and its comparison with Two-Dimensional Ultrasonography

摘要 目的本研究旨在比较彩色多普勒超声(CDFI)和二维超声(2DUS)在诊断视网膜脱离(RD)中的效能,进一步明确CDFI在这些眼病诊断中的临床应用价值。方法采用回顾性分析方法,收集2020年1月至2024年1月期间152例怀疑RD的患者数据。所有患者均进行了CDFI和2DUS检查,以术中所见作为RD最后确诊依据,将检查结果与术中结果对照,以评估每种超声技术的诊断准确性。使用卡方检验和Cohen's kappa系数分析两种超声方法的一致性和效能。结果所有152例患者均确诊RD,彩超在视网膜脱离的敏感性为98.03%,显著高于B超的89.47%,卡方值为8.08,P值为0.0045,表明两者在视网膜脱离诊断中存在显著差异。在脉络膜脱离检测中,彩超的敏感性为93.75%,同样高于B超的56.25%,卡方值为4.17,P值为0.0412。Kappa系数分析显示,彩超和B超在视网膜脱离诊断中的一致性为0.388,在脉络膜脱离诊断中的一致性为0.375,均为一般的一致性。结论彩色多普勒超声(CDFI)和B超在诊断视网膜脱离(RD)和脉络膜脱离(CD)方面均具有重要临床价值。CDFI通过提供血流信号和血管结构的可视化,展示了更高的敏感度和特异性,特别是在检测脉络膜脱离时显示出明显优势。CDFI在综合诊断性能上更优,可以更准确地评估视网膜和脉络膜的病变情况。 Objective This study aims to compare the efficacy of Color Doppler Ultrasonography(CDFI)and Two-Dimensional Ultrasonography(2DUS)in diagnosing Retinal Detachment(RD)and to further clarify the clinical application value of CDFI in diagnosing these ocular diseases.Methods A retrospective analysis was conducted on data collected from 152 patients suspected of having RD between January 2020 and January 2024.All patients underwent both CDFI and 2DUS examinations,using intraoperative findings as the"gold standard"for RD diagnosis,and the results of ultrasound examinationwere compared with surgical findings to assess the diagnostic accuracy of each ultrasound technique.Chi-square tests and Cohen's kappa coefficients were used to analyze the consistency and effectiveness of the two ultrasound methods.Results All 152 patients were diagnosed with RD based on intraoperative findings.CDFI had a detection rate of 98.03%for RD,significantly higher than the 89.47%detection rate of 2DUS,with a chi-square value of 8.08 and a p-value of 0.0045,indicating a significant difference between the two methods in diagnosing RD.For Choroidal Detachment(CD),CDFI had a detection rate of 93.75%,also higher than the 56.25%detection rate of 2DUS,with a chisquare value of 4.17 and a p-value of 0.0412.Kappa coefficient analysis showed a consistency of 0.388 between CDFI and 2DUS in diagnosing RD,and 0.375 in diagnosing CD,both indicating moderate consistency.Conclusion Color Doppler Ultrasonography(CDFI)and 2DUS both have significant clinical value in diagnosing Retinal Detachment(RD)and Choroidal Detachment(CD).CDFI,by providing blood flow signals and vascular structure visualization,demonstrated higher sensitivity and specificity,especially in detecting CD.CDFI proved to be superior in overall diagnostic performance,offering more accurate assessments of retinal and choroidal lesions.
