摘要
[目的]分析2018—2023年内蒙古自治区农村上消化道癌早诊早治项目筛查结果。[方法]2018—2023年在内蒙古自治区4个项目点采用整群抽样方法,选取40~69岁居民进行上消化道内镜检查,对发现的可干预的癌前病变及癌症患者进行及时治疗,并定期随访,所有病变的确诊均以病理学检查为依据。[结果]2018—2023年,内蒙古自治区合计完成上消化道内镜筛查14492人次,检出病例221例,检出率为1.52%,早期病例109例,早诊率为49.32%,治疗病例160例,治疗率为72.40%。2018—2023年间检出率、早诊率和治疗率变化趋势均无统计学意义(P均>0.05),胃部检出率、早诊率和治疗率均高于食管和贲门部位(P均<0.001)。[结论]内蒙古自治区农村上消化道癌人群筛查能够发现早期癌症患者,病变以胃为主,但早诊率和治疗率偏低,需进一步加强技术培训和宣传教育,提高筛查效果。
[Purpose]To analyze the results of upper gastrointestinal cancer screening in rural areas of Inner Mongolia from 2018 to 2023.[Methods]Upper gastrointestinal cancer screening program was conducted among residents aged 40~69 years old from 4 rural project sites in Inner Mongolia selected by cluster sampling method from 2018 to 2023.The detected malignant tumors and precancerous lesions were treated and confirmed by pathological examination.[Results]A total of 14492 residents were screened for upper gastrointestinal cancers in rural areas of Inner Mongolia from 2018 to 2023.The detection rate,early diagnosis rate and treatment rate were 1.52%,49.32%and 72.40%,respectively.There was no significant difference in detection rate,early diagnosis rate and treatment rate for different years(all P>0.05).The detection rate,early diagnosis rate,and treatment rate in the stomach were higher than those in the esophagus and cardia(all P<0.001).[Conclusion]The upper gastrointestinal cancer screening program can detect early-stage cancer,mainly in the stomach,but the early diagnosis rate and treatment rate are low in rural areas of Inner Mongolia,so it is necessary to further strengthen technical training and publicity and education to improve the screening effect.
