钛捆绑带联合髋关节置换术治疗股骨粗隆间骨折的效果及对患者髋关节功能的影响

Therapeutic effect of titanium binding tape combined with hip arthroplasty on intertrochanteric fracture of femur and its influence on hip joint function

目的:探讨钛捆绑带联合髋关节置换术治疗股骨粗隆间骨折(ITF)的效果及对患者髋关节功能的影响。方法:队列研究。抽取2022年1月至2022年12月许昌医院收治的ITF患者62例,根据随机数字表法分为对照组和研究组,每组31例。对照组采用钢丝环形捆扎联合髋关节置换术治疗,研究组采用钛捆绑带联合髋关节置换术治疗。比较两组手术相关指标、疼痛程度[视觉模拟评分法(VAS)评分]、髋关节功能[Harris髋关节评分系统(HHS)评分]、临床疗效及并发症发生率。结果:研究组手术时间、首次下床活动时间、住院时间、骨折愈合时间均短于对照组(P均<0.05),术中出血量少于对照组(P<0.05)。术后1周及术后1、3、6个月,两组VAS评分均较术前降低(P均<0.05),HHS评分均较术前升高(P均<0.05);术后1、3、6个月,研究组VAS评分低于对照组(P<0.05);术后3、6个月,研究组HHS评分高于对照组(P<0.05)。研究组治疗总有效率(93.55%,29/31)高于对照组(74.19%,23/31),P<0.05。研究组并发症发生率(3.23%,1/31)低于对照组(19.35%,6/31),P<0.05。结论:钛捆绑带联合髋关节置换术治疗ITF患者,可减轻患者疼痛,提高髋关节功能和治疗效果,降低并发症发生率。

Objective:To investigate the effect of titanium binding tape combined with hip arthroplasty on intertrochanteric fracture of femur(ITF)and its influence on hip function.Methods:A total of 62 patients with ITF admitted to Xuchang Hospital from January 2022 to December 2022 were selected for the cohort study,and they were divided into a control group and a study group using a random number table method,with 31 cases in each group.The control group was treated by steel wire circular tying combined with hip arthroplasty,the study group was treated by titanium binding tape combined with hip arthroplasty.The surgical related indicators,pain level evaluated by visual analogue scale(VAS)score,hip joint function assessed by Harris hip score(HHS),clinical efficacy,and incidence of complications were compared between the two groups.Results:The surgical time,first time out of bed activity,hospital stay,and fracture healing time of the study group were shorter than those of the control group(all P<0.05),and the intraoperative bleeding was less than that of the control group(P<0.05).One week,1 month,3 months and 6 months after surgery the VAS scores of both groups decreased compared with preoperative levels(all P<0.05),while the HHS scores increased compared with preoperative levels in both groups(all P<0.05).And the VAS score of the study group was lower than that of the control group 1,3,and 6 months after surgery(P<0.05),while the HHS score of the study group was higher than that of the control group 3 and 6 months after surgery(P<0.05).The total effective rate of treatment in the study group(93.55%,29/31)was higher than that in the control group(74.19%,23/31),P<0.05.The incidence of complications in the study group(3.23%,1/31)was lower than that in the control group(19.35%,6/31),P<0.05.Conclusions:Titanium binding tape combined with hip arthroplasty can reduce pain,improve hip function and therapeutic efficacy,and reduce complications.
