鼻内镜单侧鼻后神经丛切断术在难治性AR中应用及RQLQ分析

Application and RQLQ analysis of nasal endoscopic unilateral posterior nasal plexus resection in refractory AR

目的 探究鼻内镜单侧鼻后神经丛切断术在难治性变异性鼻炎(allergic rhinitis,AR)中的应用及RQLQ分析.方法 于2022年1月-2023年12月选取青海红十字医院耳鼻喉科难治性AR患者90例,随机分为两组,每组45例.对照组行鼻内镜下翼管神经切断术,观察组行鼻内镜单侧鼻后神经丛切断术,观察两组手术前后鼻塞、流涕、鼻痒、喷嚏等疾病症状,应用视觉模拟评分法(Visual Analogue Scale,VAS)对比两组手术前后疼痛情况,对比两组术后发生鼻出血、眼干、头痛、硬腭麻木等不良反应的发生率,应用过敏性鼻炎生活质量测评量表(Rhinocoujunctivitis Quality of life QuestionARre,RQLQ)评估两组手术前后生活质量.结果 与对照组相比,干预后观察组症状评分、VAS评分、不良反应发生率、RQLQ评分降低(P<0.05).结论 鼻内镜单侧鼻后神经丛切断术应用于难治性AR患者中,可改善患者疾病症状,减轻疼痛反应,降低不良反应发生率,提高生活质量,临床效果显著.

Objective To explore the application and RQLQ analysis of endoscopic unilateral posterior nasal plexus resection in refractory variant rhinitis(AR).Methods From January 2022 to December 2023,90 patients with refractory AR in the Otolaryngology Department of Qinghai Red Cross Hospital were selected and randomly divided into two groups,with 45 cases in each group.The control group underwent endoscopic resection of the pterygoid canal nerve,while the observation group underwent unilateral endoscopic resection of the posterior nasal plexus.Symptoms of nasal congestion,runny nose,nasal itching,and sneezing were observed before and after surgery in both groups.Visual Analog Scale(VAS)was used to compare the pain situation before and after surgery in the two groups,and the incidence of adverse reactions such as nosebleeds,dry eyes,headaches,and hard palate numbness after surgery was compared between the two groups,compare the quality of life before and after surgery between two groups using the Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ)score.Results Compared with the control group,the observation group showed a decrease in symptom score,VAS score,adverse reaction rate,and RQLQ score after intervention(P<0.05).Conc lus ion The application of endoscopic unilateral posterior nasal plexus resection in refractory AR patients can improve disease symptoms,reduce pain reactions,reduce the incidence of adverse reactions,improve quality of life,and have significant clinical effects.
