人工剥膜联合催产素引产对剖宫产术后阴道试产妊娠结局影响:一项回顾性研究

The effect of artificial membrane stripping combined with oxytocin induction of labor on the pregnancy outcome of TOLAC after cesarean section:a retrospective study

目的 分析人工剥膜联合催产素(OXT)引产对剖宫产(CS)术后阴道试产(TOLAC)妊娠结局的影响.方法 按回顾性分析法选择2023年4月-2024年4月新疆生产建设兵团第二师库尔勒医院产科TOLAC的100例孕妇作为此次研究对象,按催产方式不同将其分为两组,每组各50例,对照组实施OXT引产,试验组予以人工剥膜联合OXT引产,分析两组分娩结局、产程时间、出血量、住院时间以及新生儿Apgar评分.结果 试验组不良状况总发生率、CS率分别为2.00%、6.00%,明显低于对照组的20.00%、58.00%(P<0.05);试验组第一产程、第二产程、第三产程、总产程时间均明显短于对照组(P<0.05);试验组出血量明显少于对照组,住院时间明显短于对照组,且新生儿Apgar评分明显高于对照组(P<0.05).结论 TOLAC产妇实施人工剥膜联合OXT引产可获得较高价值,不仅能够缩短产程时间、住院时间,还能够减少出血量,改善妊娠结局.

Objective To analyze the effect of manual stripping of membranes combined with oxytocin(OxT)induction of labor on the pregnancy outcome of trial of labor vaginally after cesarean section(CS)(TOLAC).Methods According to the retrospective analysis method,10O cases of pregnant women with TOLAC in the obstetrics department of Korla Hospital of the Second Division of Xinjiang Production and Construction Corps(XPC)were selected as the subjects of this study from April 2023 to April 2024,and were divided into two groups of 50 cases each according to the different methods of oxytocin induction;the control group was induced by OxT,and the experimental group was induced by artificial stripping of membranes combined with OxT,and the labor outcomes of the two groups were analyzed the outcome of labor,duration of labor,hemorrhage,length of hospital stay,and neonatal Apgar score were analyzed.Results The adverse condition and CS rate of the experimental group were 2.00%and 6.00%respectively,which were significantly lower than that of the control group,which were 20.00%and 58.00%(P<0.05);the duration of the first stage of labor,the second stage of labor,the third stage of labor,and the total duration of labor of the experimental group were all significantly shorter than that of the control group(P<0.05);the amount of hemorrhage of the experimental group was significantly less than that of the control group,and the length of hospitalization of the experimental group was significantly shorter than that of the control group;the Apgar score of the newborn was significantly higher than that of the control group(P<0.05).Conclusion The implementation of artificial membrane peeling combined with OXT induction of labor in TOLAC mothers can obtain high value,which can not only shorten the duration of labor and hospitalization time,but also reduce bleeding and improve the pregnancy outcome.
