Cys C、25-(OH)D_(3)及CRP表达与社区老年2型糖尿病患者衰弱的相关性研究

Study on correlation of Cys C,25-(OH)D_(3) and CRP expression with frailty in elderly patients with type 2 diabetes mellitus in the community

摘要 目的探讨胱抑素C(Cys C)、25-羟维生素D_(3)[25-(OH)D_(3)]及C反应蛋白(CRP)的表达与社区老年2型糖尿病患者衰弱的相关性。方法选择社区老年2型糖尿病患者为研究对象,通过Fried表型衰弱量表及FRAIL衰弱量表对患者进行衰弱性筛查,收集其中100例无衰弱者为对照组,100例有衰弱者为研究组。采用全自动湿化学法测定患者血清Cys C、CRP含量,采用全自动化学发光法检测患者血浆25-(OH)D_(3)含量。分析血清Cys C、CRP、血浆25-(OH)D_(3)在2型糖尿病中的表达,通过Pearson统计软件对血清Cys C、CRP、血浆25-(OH)D_(3)与患者衰弱相关性进行研究。结果研究组血清Cys C含量(2.89±0.78)mg/L显著高于对照组的(0.84±0.16)mg/L(P<0.05);研究组血清CRP含量(13.96±2.58)mg/L显著高于对照组的(7.58±1.65)mg/L(P<0.05);研究组血浆25-(OH)D_(3)含量(22.65±4.74)ng/ml显著低于对照组的(47.58±8.76)ng/ml(P<0.05)。经Pearson相关性分析结果显示:血清Cys C含量越高,其衰弱越显著,Cys C含量与患者衰弱呈显著正相关(r=0.625,P<0.05);血清CRP含量与患者衰弱呈显著正相关(r=0.583,P<0.05);血浆25-(OH)D_(3)含量与衰弱呈显著负相关(r=-0.603,P<0.05)。结论2型糖尿病衰弱型患者存在不同程度的肾功能损害、非特异性炎症表达以及25-(OH)D_(3)不足。 Objective To explore the correlation of cystatin C(Cys C),25-hydroxyvitamin D_(3)[25-(OH)D_(3)]and C-reactive protein(CRP)expression with frailty in elderly patients with type 2 diabetes mellitus in the community.Methods Patients with type 2 diabetes mellitus in the community were selected as the research subjects,and the frailty of the patients was screened by the Fried Frailty Scale and the FRAIL Frailty Scale,among which 100 patients without frailty were collected as the control group and 100 cases with frailty as the study group.The serum Cys C and CRP contents of the two groups were determined by automatic wet chemistry method,and the plasma 25-(OH)D_(3) content of the patients was detected by automatic chemiluminescence method.The expression of serum Cys C,CRP and plasma 25-(OH)D_(3) in type 2 diabetes mellitus was analyzed,and the correlation of serum Cys C,CRP and plasma 25-(OH)D_(3) and patient frailty was studied by Pearson statistical software.Results The serum Cys C content of(2.89±0.78)mg/L in the study group was significantly higher than(0.84±0.16)mg/L in the control group(P<0.05).The serum CRP level of(13.96±2.58)mg/L in the study group was significantly higher than(7.58±1.65)mg/L in the control group(P<0.05).The plasma 25-(OH)D_(3) content of(22.65±4.74)ng/ml in the study group was significantly lower than(47.58±8.76)ng/ml in the control group(P<0.05).Pearson's correlation analysis showed that the higher the serum Cys C content,the more significant the frailty,and the Cys C content was significantly and positively correlated with the patients'debilitation(r=0.625,P<0.05).Serum CRP levels was significantly and positively correlated with patients'frailty(r=0.583,P<0.05).Plasma 25-(OH)D_(3) level was negatively correlated with patients'debility(r=-0.603,P<0.05).Conclusion Patients with type 2 diabetes mellitus and frailty have varying degrees of renal impairment and non-specific inflammatory expression,and 25-(OH)D_(3) deficiency.
