强脉冲光联合重组贻贝粘蛋白水凝胶敷料治疗面部激素依赖性皮炎的疗效及对皮肤屏障功能指标的影响分析

摘要 目的观察强脉冲光联合重组贻贝粘蛋白水凝胶敷料治疗面部激素依赖性皮炎患者的临床疗效及对皮肤屏障功能的影响。方法纳入72例面部激素依赖性皮炎患者作为观察对象,随机分为观察组(37例)和对照组(35例)。观察组使用强脉冲光联合重组贻贝粘蛋白水凝胶敷料治疗,对照组使用强脉冲光治疗。比较两组患者皮肤屏障功能(皮肤油脂含量、皮肤含水量、经表皮水分丢失量),面部自觉症状和面部客观体征评分,临床疗效和安全性。结果治疗后,两组患者皮肤油脂含量、皮肤含水量、经表皮水分丢失量较治疗前明显改善,且观察组患者皮肤油脂含量(115.53±9.23)μg/cm^(2)和皮肤含水量(35.55±4.48)%较对照组患者的(102.77±8.26)μg/cm^(2)、(28.85±3.76)%更高,经表皮水分丢失量(13.82±2.29)g/(cm^(2)·h)较对照组患者的(17.78±3.57)g/(cm^(2)·h)更低,差异有统计学意义(P<0.05)。治疗后,两组患者的面部自觉症状评分和面部客观体征评分较治疗前显著降低,且观察组患者面部自觉症状评分(1.02±0.30)分和面部客观体征评分(3.10±1.15)分较对照组患者的(4.45±1.11)、(7.89±1.22)分更低,差异有统计学意义(P<0.05)。治疗后,观察组患者的痊愈率48.65%、总有效率91.89%均较对照组患者的22.86%、71.43%更高,差异有统计学意义(P<0.05)。观察组患者的不良事件发生率5.41%较对照组患者的22.86%更低,差异有统计学意义(P<0.05)。结论强脉冲光联合重组贻贝粘蛋白水凝胶敷料可有效改善面部激素依赖性皮炎患者临床症状,对皮肤屏障功能指标具有改善作用,安全有效,值得临床推广使用。 Objective To observe the clinical efficacy of intense pulsed light combined with recombinant mussel mucin hydrogel dressing in the treatment of facial hormone-dependent dermatitis and its influence on skin barrier function.Methods A total of 72 patients with facial hormone-dependent dermatitis were included and randomly divided into an observation group(37 cases)and a control group(35 cases).The observation group was treated with intense pulsed light combined with recombinant mussel mucin hydrogel dressing,while the control group was treated with intense pulsed light.The skin barrier function(skin oil content,skin water content,transepidermal water loss),facial self-conscious symptom score and facial objective sign score,clinical efficacy and safety were compared between the two groups.Results After treatment,the skin oil content,skin water content and transepidermal water loss were significantly improved in both groups compared with those before treatment;the observation group had skin oil content of(115.53±9.23)μg/cm^(2)and skin water content of(35.55±4.48)%,which were higher than(102.77±8.26)μg/cm^(2)and(28.85±3.76)%in the control group;the transepidermal water loss of(13.82±2.29)g/(cm^(2)·h)in the observation group was lower than(17.78±3.57)g/(cm^(2)·h)in the control group;the difference was statistically significant(P<0.05).After treatment,the facial self-conscious symptom score and facial objective sign score of the two groups were significantly lower than those before treatment;the observation group had facial self-conscious symptom score of(1.02±0.30)points and facial objective sign score of(3.10±1.15)points,which were lower than(4.45±1.11)and(7.89±1.22)points in the control group;the difference was statistically significant(P<0.05).After treatment,the observation group had cure rate of 48.65%and total effective rate of 91.89%,which were higher than 22.86%and 71.43%in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse events in the observation group was 5.41%,which was lower than 22.86%in the control group,and the difference was statistically significant(P<0.05).Conclusion Intense pulsed light combined with recombinant mussel mucin hydrogel dressing can effectively improve the clinical symptoms of patients with facial hormone-dependent dermatitis,improve the indexes of skin barrier function.It is safe and effective,and is worthy of clinical promotion.
