动态心电图对冠心病患者心律失常的诊断效果观察

Observation of the diagnostic effect of Holter electrocardiogram on arrhythmia in patients with coronary heart disease

目的观察动态心电图对冠心病患者心律失常的诊断效果。方法选取经心脏生理检查明确为冠心病的患者70例纳入研究,均实施常规心电图检查及动态心电图检查。比较常规心电图与动态心电图对冠心病及心律失常的阳性检出率,分析常规心电图与动态心电图对不同类型房性心律失常和室性心律失常的阳性检出情况。结果动态心电图的冠心病阳性检出率94.29%(66/70)显著高于常规心电图的72.86%(51/70)(χ^(2)=11.706,P<0.05)。动态心电图对心律失常阳性检出率84.29%、房性心律失常阳性检出率95.12%、室性心律失常阳性检出率68.97%均高于常规心电图的44.29%、51.22%、34.48%(P<0.05)。动态心电图对阵发性心房扑动(房扑)、心房颤动(房颤)阳性检出率为100.00%,房性期前收缩成对阳性检出率为100.00%,房性期前收缩早发阳性检出率为88.89%,房性期前收缩二、三联律阳性检出率为85.71%,均高于常规心电图的45.45%、57.14%、44.44%、57.14%。动态心电图对室性期前收缩阳性检出率72.73%、室性心动过速阳性检出率71.43%、心室扑动阳性检出率60.00%、心室颤动阳性检出率66.67%均高于常规心电图的45.45%、28.57%、20.00%、33.33%。结论动态心电图可提高冠心病患者房性心律失常以及室性心律失常阳性检出率,尤其是在检查不同类型房性心律失常方面具有较高应用价值,值得临床推广。

Objective To observe the diagnostic effect of Holter electrocardiogram on arrhythmia in patients with coronary heart disease.Methods A total of 70 patients with coronary heart disease confirmed by cardiac physiological examination were included in the study.Routine electrocardiogram and Holter electrocardiogram were performed in all patients.The positive detection rate of coronary heart disease and arrhythmia by routine electrocardiogram and Holter electrocardiogram was compared,and the positive detection of atrial arrhythmia and ventricular arrhythmia by routine electrocardiogram and Holter electrocardiogram was statistically analyzed.Results The positive detection rate of coronary artery disease by Holter electrocardiogram was 94.29%(66/70),which was significantly higher than 72.86%(51/70)by routine electrocardiogram(χ^(2)=11.706,P<0.05).The positive detection rates of Holter electrocardiogram for arrhythmia,atrial arrhythmia,and ventricular arrhythmia were 84.29%,95.12%,and 68.97%,which were higher than 44.29%,51.22%,and 34.48%of routine electrocardiogram(P<0.05).The positive detection rate of Holter electrocardiogram for paroxysmal atrial flutter/fibrillation was 100.00%,the positive detection rate for paired premature atrial contraction was 100.00%,the positive detection rate for preatrial systole early was 88.89%,and the positive rate for concealed atrial bigeminy/trigeminy was 85.71%,which were higher than 45.45%,57.14%,44.44%and 57.14%of routine electrocardiogram.The positive detection rates of Holter electrocardiogram for premature ventricular contraction was 72.73%,the positive detection rate for ventricular tachycardia was 71.43%,the positive detection rate for ventricular flutter was 60.00%,and the positive detection rate for ventricular fibrillation was 66.67%,which were higher than 45.45%,28.57%,20.00%,and 33.33%of routine electrocardiogram.Conclusion Holter electrocardiogram can improve the positive detection rate of atrial arrhythmia and ventricular arrhythmia in patients with coronary heart disease,especially in the detection of different types of atrial arrhythmia,which is of high value and worthy of clinical promotion.
