围绝经期异常子宫出血患者应用米非司酮片联合复方炔诺酮片治疗的效果观察

Effect observation of mifepristone tablets combined with compound norethisterone tablets on patients with abnormal uterine bleeding during the perimenopause

目的分析围绝经期异常子宫出血患者应用米非司酮片联合复方炔诺酮片治疗的效果。方法50例围绝经期异常子宫出血患者,通过随机数字表法将其分为对照组和观察组,每组25例。对照组实施米非司酮片治疗,观察组在对照组基础上实施复方炔诺酮片治疗。对比两组患者治疗效果、止血情况、不良反应发生情况及治疗前后血清性激素水平。结果观察组患者治疗总有效率96.00%显著高于对照组的72.00%(P<0.05)。观察组患者治疗后出血量(104.55±20.34)ml少于对照组的(132.63±21.59)ml,完全止血时间(6.25±1.66)d以及控制出血时间(24.79±5.11)h均短于对照组的(10.89±2.89)d、(31.24±3.08)h(P<0.05)。两组患者治疗后睾酮、黄体生成素、雌二醇、卵泡刺激素均较治疗前显著改善,且观察组患者治疗后睾酮(1.46±0.88)nmol/L、黄体生成素(5.21±3.65)U/L、卵泡刺激素(5.26±0.19)U/L均低于对照组的(2.33±1.06)nmol/L、(9.84±4.37)U/L、(6.32±0.34)U/L,雌二醇(92.69±15.50)pg/ml高于对照组的(81.63±11.47)pg/ml(P<0.05)。观察组与对照组患者治疗后不良反应发生率对比无统计学意义(P>0.05)。结论对围绝经期异常子宫出血患者实施米非司酮片联合复方炔诺酮片治疗具有显著的临床效果,能够有效减少患者出血量,缩短止血时间,还能够改善性激素水平,并且安全性较高,建议在临床中进一步推广。

Objective To analyze the effect of mifepristone tablets combined with compound norethisterone tablets on patients with abnormal uterine bleeding during the perimenopause.Methods A total of 50 patients with abnormal uterine bleeding during the perimenopause were divided into a control group and an observation group according to random numerical table method,with 25 patients in each group.The control group received mifepristone tablets for treatment,while the observation group received compound norethisterone tablets for treatment on the basis of the control group.The treatment effect,hemostasis,occurrence of adverse reactions and serum sex hormone levels before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 96.00%,which was significantly higher than 72.00%of the control group(P<0.05).After treatment,the bleeding volume of(104.55±20.34)ml in the observation group was less than(132.63±21.59)ml in the control group;the observation group had complete hemostasis time of(6.25±1.66)d and bleeding control time of(24.79±5.11)h,which were shorter than(10.89±2.89)d and(31.24±3.08)h in the control group(P<0.05).After treatment,the testosterone,luteinizing hormone,estradiol and follicle stimulating hormone in both groups were significantly improved compared with those before treatment;in the observation group,the testosterone was(1.46±0.88)nmol/L,the luteinizing hormone was(5.21±3.65)U/L and the follicle stimulating hormone was(5.26±0.19)U/L,which were lower than(2.33±1.06)nmol/L,(9.84±4.37)U/L and(6.32±0.34)U/L in the control group;the estradiol of(92.69±15.50)pg/ml in the observation group was higher than(81.63±11.47)pg/ml in the control group(P<0.05).There was no statistical significance in the incidence of adverse reactions after treatment between the observation group and the control group(P>0.05).Conclusion The combination of mifepristone tablets and compound norethisterone tablets has a significant clinical effect in patients with abnormal uterine bleeding during the perimenopause.It can not only shorten the bleeding volume and hemostasis time of patients,but also improve their sex hormone level.It has high safety,and is recommended to further promote it in clinical practice.
