摘要
目的:在DRG改革背景下,分析重庆市主城区公立医院推行DRG支付方式改革相关政策与医疗行为关系,为支付方式改革提供合理建议。方法:对重庆主城区10家公立医院的572名医护人员开展问卷研究,采用Pearson相关性和典型相关分析研究公立医院推行DRG改革的相关政策与医疗行为之间的相关性。结果:公立医院推行DRG改革相关政策与积极医疗行为(P<0.05)呈正相关,与消极医疗行为(P<0.05)呈负相关。公立医院推行DRG改革的相关政策与医疗行为之间的典型相关系数达到0.792(P<0.001),表现为两者之间关系较为密切,公立医院推行DRG相关政策对医疗行为的影响作用大于医疗行为对公立医院推行DRG改革相关政策的影响作用。结论:公立医院推行DRG改革相关政策与医疗行为具有较高相关性,医院在制定DRG改革相关政策时应予以重视,以提供优质高效医疗行为。
Objective:To analyze the relationship between the policies related to the implementation of DRG payment reform in public hospitals in the main urban areas of Chongqing and medical behavior,and to provide reasonable suggestions for the reform of the payment system.Methods:A questionnaire survey was conducted with 572 medical staff from 10 public hospitals in the main urban areas of Chongqing.Pearson correlation and canonical correlation analysis were used to study the correlation between the policies related to DRG reform and medical behavior in public hospitals.Results:Policies related to the implementation of DRG reform in public hospitals were positively correlated with positive medical behavior(P<0.05)and negatively correlated with negative medical behavior(P<0.05).The canonical correlation coefficient between DRG-related policies and medical behavior reached 0.792(P<0.001),indicating a close relationship between the two,with the influence of DRG-related policies on medical behavior being greater than the reverse.Conclusion:There is a high correlation between DRG-related policies and medical behavior in public hospitals.Hospitals should take this into account when formulating DRG-related policies to ensure the provision of high-quality and efficient medical care.
