胰岛素联合低分子肝素对高脂血症性急性胰腺炎的影响

Effect of Insulin Combined With Low Molecular Weight Heparin on Hyperlipidemic Acute Pancreatitis

目的探讨胰岛素联合低分子肝素治疗高脂血症性急性胰腺炎(hyperlipidemic pancreatitis,HLAP)患者的临床效果。方法选取2021年2月—2023年3月厦门市第三医院筛选入组的128例HLAP患者为对象,以信封法分为对照组与研究组,各64例。对照组给予低分子肝素治疗,研究组给予胰岛素联合低分子肝素治疗。比较2组HLAP患者的临床治疗情况、氧化应激反应、炎症反应以及发生不良反应发生率。结果研究组治疗总有效率为95.31%,高于对照组的84.38%,差异有统计学意义(P<0.05);治疗后,研究组超氧化物歧化酶(superoxide dismutase,SOD)指数高于对照组,丙二醛(malondialdehyde,MDA)及氧化型谷胱甘肽(glutathione disulfide,GSSG)指数均低于对照组,差异有统计学意义(P<0.05);研究组炎症因子水平低于对照组,差异有统计学意义(P<0.05);研究组不良反应总发生率与对照组比较,差异无统计学意义(P>0.05)。结论胰岛素联合低分子肝素治疗HLAP患者可提高临床治疗效率,缓解氧化应激反应,减轻炎症反应,不良反应较少。

Objective To explore the clinical effect of insulin combined with low molecular weight heparin in the treatment of hyperlipidemic acute pancreatitis(HLAP).Methods A total of 128 HLAP patients selected from the Third Hospital of Xiamen from February 2021 to March 2023 were selected as subjects and divided into control group and study group by envelope method,with 64 cases in each group.The control group was treated with low molecular weight heparin,while the study group was treated with insulin and low molecular weight heparin.The clinical treatment,oxidative stress reaction,inflammatory reaction and the incidence of adverse reactions were compared between the two groups of HLAP patients.Results The total effective rate of the study group was 95.31%,which was higher than 84.38%of the control group,the difference was statistically significant(P<0.05).After treatment,the index of superoxide dismutase(SOD)in the study group was higher than that in the control group,and the indices of malondialdehyde(MDA)and glutathione disulfide(GSSG)in the study group were lower than those in the control group,the difference was statistically significant(P<0.05).The level of inflammatory factors in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the study group and the control group(P>0.05).Conclusion The combination of insulin and low molecular weight heparin in the treatment of HLAP patients can significantly improve the treatment efficiency,alleviate oxidative stress reactions,alleviate inflammatory reactions,and have fewer adverse reactions.
