摘要
目的:探讨人工关节置换术和螺钉内固定术对老年股骨颈骨折患者的疗效。方法:将60例老年股骨颈骨折患者随机分为对照组(n=30)和研究组(n=30)。对照组采用空心螺钉内固定术,研究组接受人工关节置换术治疗。比较2组围术期手术指标、治疗效果、髋关节恢复情况、生活质量、并发症发生情况。结果:研究组手术时长、术中出血量高于对照组,卧床及住院时间低于对照组(P<0.05);研究组治疗总有效率和髋关节功能优良率均高于对照组。术后3个月,研究组GQOL-74量表各维度评分均高于对照组(P<0.05),并发症总发生率低于对照组(P>0.05)。结论:与螺钉内固定术相比,人工关节置换术治疗老年股骨颈骨折具有更好的治疗效果,有助于术后髋关节功能的恢复和生活质量的改善。
Objective:To investigate the efficacy of artificial arthroplasty and screw internal fixation in elderly patients with femoral neck fracture.Methods:Sixty elderly patients with femoral neck fractures were randomly divided into a control group(n=30)and a study group(n=30).The control group was treated with hollow screw internal fixation,and the study group was treated with artificial joint replacement.The perioperative surgical indexes,therapeutic effects,hip joint recovery,quality of life,and the occurrence of complications were compared between the two groups.Results:In the study group,the duration of surgery and the volume of intraoperative bleeding were higher than those of the control group,and the bed time and hospitalisation time were shorter than those of the control group(P<0.05).The total effective rate of treatment and the rate of excellent hip function of the study group were higher than those of the control group.At 3 months after surgery,the scores of all dimensions of GQOL-74 scale of the study group were higher than those of the control group(P<0.05),and the incidence of total complications was lower than that of the control group(P>0.05).Conclusion:Compared with screw internal fixation,artificial joint arthroplasty for the treatment of elderly femoral neck fracture has better therapeutic effect and of benefit to the recovery of hip joint function and improvement of quality of life after surgery.
