NICU出院患儿短期内非计划性再入院的危险因素分析

Risk factors for short-term unplanned readmission of children discharged from the NICU

目的探讨新生儿重症监护室(NICU)出院患儿短期内非计划性再入院的危险因素。方法前瞻性纳入2020年1月至2022年12月于安阳市妇幼保健院NICU出院的800例患儿为研究对象,所有患儿出院后接受30 d随访。设计NICU患儿基线资料调查表,统计研究所需资料及随访期间患儿再入院情况。应用单因素分析及多因素Logistic回归分析模型筛选影响NICU出院患儿再入院的影响因素。结果随访30 d内,800例患儿中再入院31例,再入院率3.88%(31/800),入院病因主要为高胆红素血症(35.48%,11/31)、肺炎(29.03%,9/31)。单因素、多因素Logistic回顾分析结果显示,前次住院合并高胆红素血症、早产儿、前次住院接受机械通气治疗可能是NICU出院患儿短期内非计划性再入院的危险因素(OR=3.72、3.09、3.04,P均<0.05)。结论NICU出院患儿短期内再入院的危险因素可能为合并高胆红素血症、早产儿、前次住院接受机械通气治疗。

ObjectiveTo investigate the risk factors for short-term unplanned readmission of children discharged from the neonatal intensive care unit(NICU).MethodsA total of 800 children discharged from the NICU of Anyang Maternal and Child Health Care Hospital from January 2020 to December 2022 were prospectively enrolled in the study.And they were followed up for 30 days after discharge.The baseline data questionnaire for NICU children was designed to collect and record the data required for statistical studies and the readmission status of them children.Univariate analysis and multivariate logistic regression analysis were used to screen the influencing factors affecting the readmission after discharge from NICU.ResultsWithin 30 days of follow-up,31 of 800 children were readmitted to the hospital,with a readmission rate of 3.88%(31/800),and the main causes of admission were hyperbilirubinemia(35.48%,11/31)and pneumonia(29.03%,9/31).Univariate and multivariate logistic retrospective analyses showed that the previous hospitalization with hyperbilirubinemia,preterm infants,and previous hospitalization with mechanical ventilation might be risk factors for short-term unplanned readmission in children discharged from the NICU(OR=3.72,3.09,3.04;all P<0.05).ConclusionsHyperbilirubinemia,preterm infants,and mechanical ventilation therapy in the previous hospitalization may be the risk factors for short-term readmission in children discharged from the NICU.
