探究初诊2型糖尿病使用达格列净联合二甲双胍的疗效

Investigate the Efficacy of Dapagliflozin Combined with Metformin in Newly Diagnosed Type 2 Diabetes

目的探究初诊2型糖尿病患者应用达格列净联合二甲双胍的临床效果。方法选取2022年7月—2023年3月湘潭市第二人民医院收治的80例初诊2型糖尿病为研究对象,根据不同治疗方法分为两组,Aca组应用二甲双胍+阿卡波糖治疗,Dap组应用二甲双胍+达格列净治疗,每组40例。对比两组患者体重指数、血糖指标(空腹血糖、餐后2 h血糖、糖化血红蛋白)、血脂指标(血清总胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇)、胰岛β细胞功能指数、胰岛素抵抗指数、不良反应(恶心、腹胀、腹泻、泌尿生殖道感染、低血糖)发生率。结果治疗后,Dap组空腹血糖、餐后2 h血糖、糖化血红蛋白、体重指数、甘油三酯、血清总胆固醇、低密度脂蛋白胆固醇、胰岛素抵抗指数均低于Aca组,差异有统计学意义(P均<0.05)。治疗后,Dap组高密度脂蛋白胆固醇、胰岛β细胞功能指数均高于Aca组,差异有统计学意义(P均<0.05)。Dap组不良反应总发生率低于Aca组,差异有统计学意义(P<0.05)。结论应用达格列净联合二甲双胍治疗初诊2型糖尿病患者,可有效降低患者血糖和血脂水平,且不良反应少。

Objective To explore the clinical effect of dapagliflozin combined with metformin in patients with newly diagnosed type 2 diabetes.Methods A total of 80 patients with newly diagnosed type 2 diabetes admitted to the Sec-ond People's Hospital of Xiangtan from July 2022 to March 2023 were selected as the research objects.According to different treatment methods,they were divided into two groups.The Aca group was treated with metformin+acarbose,and the Dap group was treated with metformin+dapagliflozin,with 40 cases in each group.The body mass index,blood glucose index(fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c),blood lipid index(serum total cholesterol,triglyceride,low density lipoprotein cholesterol,high density lipoprotein choles-terol),homeostatic model assessment of isletβcell function index,homeostatic model assessment of insulin resistance and incidence of adverse reactions(nausea,abdominal distension,diarrhea,genitourinary tract infection,hypoglyce-mia)were compared between the two groups.Results After treatment,fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,body mass index,triglyceride,serum total cholesterol,low density lipopro-tein cholesterol and homeostatic model assessment of insulin resistance in the Dap group were lower than those in the Aca group,and the differences were statistically significant(all P<0.05).After treatment,the high-density lipoprotein cholesterol and homeostatic model assessment of isletβcell function index in the Dap group were higher than those in the Aca group,and the differences were statistically significant(both P<0.05).The total incidence of adverse reactions in the Dap group was lower than that in the Aca group,and the difference was statistically significant(P<0.05).Conclusion The application of dapagliflozin combined with metformin in the treatment of newly diagnosed patients with type 2 diabetes can effectively reduce the blood glucose and blood lipid levels of patients,and has fewer adverse reac-tions.
