家庭会议对降低终末期癌症患者PI发生率的效果研究

Effect of family meetings on reducing the incidence of PI in patients with end-stage cancer

目的利用家庭会议为压力性损伤(PI)高风险终末期癌症患者创建个性化皮肤管理模式,并评价应用效果。方法选择2022年1月至2023年6月入住某三甲医院肿瘤科符合纳入标准的120例PI高风险终末期癌症患者作为研究对象,采用随机数字表法分为对照组和试验组,每组60例,对照组患者实施常规皮肤护理,试验组患者在常规皮肤护理的基础上利用家庭会议创建个性化皮肤管理模式,比较2组患者皮肤管理效果和家属满意度。结果试验组患者PI发生率(1.67%)低于对照组(13.33%),压疮愈合计分量表评分低于对照组,局部减压垫、压疮防治床垫使用率(分别为18.33%、61.67%)高于对照组(分别为5.00%、36.67%),差异均有统计学意义(P<0.05);2组患者泡沫敷料使用率(均为3.33%)比较,差异无统计学意义(P>0.05)。试验组患者家属非常满意度(100.00%)高于对照组(91.67%),差异有统计学意义(P<0.05)。结论对有PI高风险的终末期癌症患者,利用家庭会议创建个性化皮肤管理模式可降低PI发生率和皮肤损伤程度,提高局部减压垫、压疮防治床垫的使用率,提升家属满意度。

Objective To develop and evaluate the effects of a personalized skin management model for terminally ill cancer patients at high risk for Pressure Injuries(PI)using family meetings.Methods A total of 120 terminally ill cancer patients at high risk for PI,who met the inclusion criteria in department of oncology of a tertiary hospital from January 2022 to June 2023 were selected as study subjects and randomized into a control group and an experimental group,with 60 patients in each.The control group received routine skin care,while the experimental group developed a personalized skin management model through family meetings in addition to routine skin care.The effects of skin management and family satisfaction were compared between the two groups.Results The incidence of PI in the experimental group(1.67%)was lower than that in the control group(13.33%).Scores on the pressure ulcer healing chart were lower in the experimental group,and the usage rates of local pressure-reducing cushions and pressure ulcer prevention mattresses(18.33%and 61.67%,respectively)were higher than those in the control group(5.00%and 36.67%,respectively),with statistically significant differences(P<0.05).The use rates of foam dressings were the same in both groups(3.33%),showing no significant difference(P>0.05).The degree of extreme satisfaction among family members of the experimental group(100.00%)was higher than that of the control group(91.67%),showing a statistically significant difference(P<0.05).Conclusion For terminally ill cancer patients at high risk for PI,using family meetings to create a personalized skin management model can reduce the incidence and severity of PI,increase the usage rates of local pressure-reducing cushions and pressure ulcer prevention mattresses,and enhance family satisfaction.
