个体化医学营养结合胰岛素治疗GDM的疗效及对血糖水平的影响

Effect of Individualized Medical Nutrition Combined with Insulin on the Treatment of GDM and Its Influence on Blood Glucose Standard

目的探讨妊娠糖尿病治疗中应用个体化医学营养联合胰岛素的临床效果。方法方便选取2021年3月—2022年10月滕州市中心人民医院妇产科收治的77例妊娠期糖尿病(gestational diabetes mellitus,GDM)患者为研究对象,以不同治疗方法分为对照组和观察组,对照组(n=38)给予胰岛素治疗,观察组(n=39)在对照组基础上给予个体化医学营养治疗。对比两组患者血糖水平、营养指标水平及不良反应发生情况。结果治疗后,观察组糖化血红蛋白(6.17±1.06)%、空腹血糖(6.50±1.20)mmol/L、餐后2 h血糖(8.31±1.61)mmol/L均低于对照组的(7.40±1.11)%、(8.39±1.30)mmol/L、(10.41±1.54)mmol/L,差异有统计学意义(t=4.974、6.632、5.846,P均<0.05)。观察组营养指标水平优于对照组,差异有统计学意义(P<0.05)。两组患者不良反应(羊水量异常、产后出血、合并高血压)总发生率比较,差异无统计学意义(P>0.05)。结论个体化医学营养联合胰岛素治疗妊娠糖尿病效果良好,可改善营养状态,降低血糖水平。

Objective To investigate the effect of combined personalized medical nutrition plus insulin in the treatment of gestational diabetes mellitus.Methods A total of 77 patients with gestational diabetes mellitus(GDM)admitted to the Obstetrics and Gynecology Department of Tengzhou Central People's Hospital from March 2021 to October 2022 were conveniently selected as the study objects.They were divided into control group and observation group by different treatment methods.The control group(n=38)was given insulin treatment.Observation group(n=39)was given individualized medical nutrition treatment on the basis of control group.Blood glucose levels,nutrition index level and adverse reactions were compared between the two groups.Results After treatment,the glycosylated hemoglobin A1c(6.17±1.06)%,fasting blood glucose(6.50±1.20)mmol/L and 2 h postprandial blood glucose(8.31±1.61)mmol/L in the observation group were lower than those in the control group(7.40±1.11)%,(8.39±1.30)mmol/L,(10.41±1.54)mmol/L,the differences were statistically significant(t=4.974,6.632,5.846,all P<0.05).The nutritional index level of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions(abnormal amniotic fluid volume,postpartum hemorrhage,combined hypertension)between the two groups(P>0.05).Conclusion Individualized medical nutrition combined with insulin is effective in the treatment of gestational diabetes,improving nutritional status and reducing blood glucose levels.
