多巴酚丁胺联合左西孟旦在急性心力衰竭患者中的治疗效果及对血清NT-proBNP与炎症因子的影响

Efficacy of Dobutamine Combined with Levosimendan in Patients with Acute Heart Failure and its Effect on Serum NT-proBNP and Inflamma⁃tory Factors

目的探讨急性心力衰竭(acute heart failure,AHF)患者采用多巴酚丁胺与左西孟旦联合治疗的效果及对血清氨基末端脑利钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)与炎症因子的影响。方法随机选取2023年2月—2024年1月曹县人民医院收治的88例AHF患者为研究对象,根据不同治疗方法分为对照组和研究组,每组44例。对照组接受多巴酚丁胺治疗,研究组在多巴酚丁胺治疗的基础上联合使用左西孟旦。对比两组患者的临床疗效、心功能指标、血清NT-proBNP与炎症因子水平以及不良反应发生情况。结果研究组治疗总有效率[93.18%(41/44)]较对照组[75.00%(33/44)]更高,差异有统计学意义(χ^(2)=5.436,P<0.05)。治疗后,研究组心搏出量、左心室射血分数指标较对照组更高,NT-proBNP、降钙素原、C-反应蛋白水平较对照组更低,差异有统计学意义(P均<0.05)。两组患者不良反应对比,差异无统计学意义(P>0.05)。结论AHF患者采用多巴酚丁胺联合左西孟旦联合治疗的总有效率较高,可以改善心功能与血清NT-proBNP水平,抑制炎症反应。

Objective To investigate the effect of dobutamine combined with levosimendan in patients with acute heart failure(AHF)and its effect on serum N-terminal pro-brain natriuretic peptide(NT-proBNP)and inflammatory factors.Methods A total of 88 patients with AHF admitted to Caoxian People's Hospital from February 2023 to January 2024 were randomly selected and divided into control group and study group by different treatment methods,with 44 cases in each group.The control group was treated with dobutamine,and the study group was treated with levosimendan on the basis of dobutamine.The clinical efficacy,cardiac function indexes,serum NT-proBNP and inflammatory factors levels and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the study group[93.18%(41/44)]was higher than that in the control group[75.00%(33/44)],and the difference was statistically significant(χ^(2)=5.436,P<0.05).After treatment,the indexes of stroke volume and left ventricular ejection fraction in the study group were higher than those in the control group,and the levels of NT-proBNP,procalcitonin and C-reactive protein were lower than those in the control group,the differences were statistically significant(all P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The total effective rate of dobutamine combined with levosimendan in patients with AHF is higher,and it can improve cardiac function and serum NT-proBNP level,and inhibit inflammatory response.
