维生素A联合地特胰岛素-门冬胰岛素对糖尿病患儿糖脂代谢水平的影响

Effect of Vitamin A Combined with Insulin Detemir-Insulin Aspart on Glucose and Lipid Metabolism in Children with Diabetes Mellitus

目的分析维生素A联合地特胰岛素-门冬胰岛素治疗糖尿病患儿的作用。方法回顾性选取2022年10月—2024年3月晋江市医院(上海市第六人民医院福建医院)收治的120例糖尿病患儿的临床资料,根据不同治疗方式分为两组,各60例。对照组采用地特胰岛素联合门冬胰岛素治疗,研究组在对照组基础上联用维生素A治疗。比较两组糖脂代谢水平(糖化血红蛋白、空腹血糖、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇)、血糖达标时间、并发症(视网膜病变、酮症酸中毒、低血糖)发生率。结果治疗前,两组糖脂代谢水平比较,差异无统计学意义(P均>0.05);治疗后,研究组糖脂代谢水平优于对照组,差异有统计学意义(P均<0.05)。研究组血糖达标时间短于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论维生素A联合地特胰岛素-门冬胰岛素治疗糖尿病患儿,能够显著改善糖脂代谢水平,提高血糖水平,并降低并发症发生风险。

Objective To analyze the effect of vitamin A combined with insulin detemir-insulin aspart in the treat⁃ment of children with diabetes mellitus.Methods The clinical data of 120 children with diabetes admitted to Jinjiang Hospital(Shanghai Sixth People's Hospital Fujian Hospital)from October 2022 to March 2024 were retrospectively se⁃lected and divided into two groups according to different treatment methods,60 cases in each group.The control group was treated with insulin detemir combined with insulin aspart,and the study group was treated with vitamin A on the basis of the control group.The levels of glucose and lipid metabolism(glycated hemoglobin A1c,fasting blood glucose,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol),blood glucose compliance time,and the rate of complications(retinopathy,ketoacidosis,hypoglycemia)were compared between the two groups.Results Before treatment,there were no statistically significant differences in glucose and lipid metabolism between the two groups(all P>0.05).After treatment,the leves of glucolipid metabolism of study group were better than those of control group,the differences were statistically significant(all P<0.05).The time of blood glucose reaching the standard in the study group was shorter than that in the control group,the difference was statistically significant(P<0.05).The in⁃cidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Vitamin A combined with insulin detemir-insulin aspart in the treatment of chil⁃dren with diabetes mellitus can significantly improve the level of glucose and lipid metabolism,increase the level of blood glucose,and reduce the risk of complications.
