活血舒筋汤辅助治疗在人工膝关节置换术后患者中的应用

Application of Huoxue Shujin Decoction as an Adjunctive Treatment in Patients After Total Knee Arthroplasty

目的分析活血舒筋汤辅助治疗在人工膝关节置换术后患者中的应用效果。方法纳入2020年10月至2023年10月洛阳市新安县人民医院人工膝关节置换术后患者(82例)为研究样本,按随机单双数抽签法划分组别,41例单数入对照组,予常规药物配合术后康复训练;另41例双数者入研究组,在常规药物配合术后康复训练前提下予以活血舒筋汤辅助治疗。对比两组治疗前后视觉模拟评分(VAS)、治疗前后膝关节肿胀度、下肢深静脉血栓发生率及治疗前后膝关节功能评分(HSS)。结果治疗前两组疼痛VAS评分对比差异无统计学意义(P>0.05),治疗第5 d及第14 d两组疼痛VAS评分均低于治疗前,且研究组疼痛VAS评分低于对照组,差异均有统计学意义(P<0.05);治疗前两组膝关节肿胀度对比差异无统计学意义(P>0.05),治疗后两组膝关节肿胀度低于治疗前,且研究组膝关节肿胀度低于对照组,差异均有统计学意义(P<0.05);研究组下肢深静脉血栓发生率(2.44%)低于对照组(14.63%),差异具有统计学意义(P<0.05);治疗前两组膝关节功能评分对比差异无统计学意义(P>0.05),治疗后两组疼痛、功能、肌力、稳定性、活动度评分均高于治疗前,且研究组治疗后HSS各维度评分均高于对照组,差异均有统计学意义(P<0.05)。结论人工膝关节置换术后采取活血舒筋汤辅助治疗可缓解术后疼痛、肢体肿胀、预防下肢深静脉血栓,同时有利于下肢功能康复,值得应用。

Objective To analyze the effect of Huoxue Shujin Decoction as an adjunctive treatment in patients after total knee arthroplasty.Methods A total of 82 patients who underwent total knee arthroplasty at Xin’an People’s Hospital from October 2020 to October 2023 were included as research samples.They were divided into groups using a random odd-even number method.Forty-one odd-numbered patients were assigned to the control group and received conventional medication combined with postoperative rehabilitation training.The other 41 even-numbered patients were assigned to the study group and received Huoxue Shujin Decoction in addition to conventional medication and postoperative rehabilitation training.The visual analog scale(VAS)scores,knee joint swelling,incidence of deep vein thrombosis of the lower limbs,and Hospital for special surgery(HSS)before and after treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in VAS scores between the two groups(P>0.05).On the 5th and 14th days of treatment,the VAS scores of both groups were lower those before treatment,and the VAS scores of the study group were lower than those of the control group,with statistically significant differences(P<0.05).Before treatment,there was no statistically significant difference in knee joint swelling between the two groups(P>0.05).After treatment,the knee joint swelling in both groups was lower than before treatment,and the knee joint swelling in the study group was lower than that in the control group,with statistically significant differences(P<0.05).The incidence of deep vein thrombosis in the lower limbs in the study group(2.44%)was lower than that in the control group(14.63%),with a statistically significant difference(P<0.05).Before treatment,there was no statistically significant difference in knee joint function scores between the two groups(P>0.05).After treatment,the scores for pain,function,muscle strength,stability,and range of motion in both groups were higher than before treatment,and the HSS scores in the study group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion The use of Huoxue Shujin Decoction as an adjunctive treatment after total knee arthroplasty can alleviate postoperative pain and limb swelling,prevent deep vein thrombosis of the lower limbs,and promote the recovery of lower limb function.It is worth applying.
