牙髓血运重建术治疗年轻恒牙根尖周炎的临床观察

Clinical observation of pulp revascularization for the treatment of periapical periodontitis in young permanent teeth

目的 探讨牙髓血运重建术用于年轻恒牙根尖周炎患者治疗的临床价值。方法 研究对象为60例年轻恒牙根尖周炎患者,采集2020年1月至2022年9月采集桂林市口腔医院病例收治入档,基于随机数表法分为对照组和观察组,每组各30例,相对应治疗方案分别为根尖诱导成形术、牙髓血运重建术;观察比较两组的临床疗效、患牙疼痛评分、牙冠根比、牙根管壁厚度、牙根和根尖孔改变情况、并发症发生率。结果 与对照组比较,观察组临床总有效率更高(P <0.05);治疗后较之前测评视觉模拟评分法(VAS)的结果,两组均降低,且观察组更为突出(P <0.05);治疗后1年较之前比较,两组的患牙冠根比、牙根管壁厚度均提高,且观察组更为突出(P <0.05);治疗后,观察组牙根长度、牙根面积、根尖孔大小的改变更优(P <0.05);两组并发症少,差异无统计学意义(P> 0.05)。结论 年轻恒牙根尖周病变患者实施牙髓血运重建治疗,疗效有保证,能显著减轻牙疼痛症状,促进牙根生长和牙髓再生。

Objective To explore the clinical value of dental pulp revascularization technique in the treatment of periapical diseases of permanent teeth in the elderly.Methods Participants in the study were 60 young patients with periapical lesions of permanent teeth,who were admitted to Guilin Stomatological Hospital from January 2020 to September 2022.They were divided into control group and observation group by random number table method,with 30 c ases in each group,and the corresponding treatment schemes were apical induction molding and pulp revascularization.The clinical efficacy,dental pain score,crown-root ratio,root canal wall thickness,root and apical foramen changes and the incidence of complications were observed and compared between the two groups.Results Compared with the control group,the total clinical effective rate in the observation group was(P<0.05).After treatment,the visual analogue scale(VAS)was lower in both groups than before,especially in the observation group(P<0.05).One year after treatment,the ratio of crown to root and the thickness of root canal wall in the two groups increased,especially in the observation group(P<0.05).After treatment,the changes of root length,root area and apical foramen size in the observation group were better than before(P<0.05).There was no significant difference in complications between the two groups(P>0.05).Conclusion The treatment of pulp revascularization in patients with periapical lesions of young permanent teeth is effective and safe,which can significantly relieve the symptoms of tooth pain and promote root growth and pulp regeneration.
