山东省血液净化护士临床决策能力现状及影响因素分析

Current Status of Clinical Decision-making Ability of Nurses Engaged in Blood Purification and Analysis of Influencing Factors in Shandong Province

目的:调查山东省血液净化中心护士临床决策能力现状并分析其影响因素,以期为血液净化护士临床决策能力的提高提供科学依据。方法:选取651名血液净化护士作为研究对象,采用一般资料调查问卷、护理临床决策量表和血液净化护士专业胜任力问卷作为测评工具进行调查,单因素分析、Spearman相关分析描述血液净化护士临床决策能力现状,并分析其影响因素。结果:血液净化护士临床决策能力总分为122(116,130)分,处于中等及以上水平;血液净化护士胜任力总分为90(66,115)分,处于中等及以上水平;性别、受教育程度、护龄、专业技术职称、人事关系、职务和自我感知护患关系是否平等是血液净化护士临床决策能力的主要因素,血液净化护士胜任力总分和各维度与临床决策能力总分及各维度呈正相关(P<0.05)。结论:为了提高血液净化护士的临床决策能力,除了提升护士的受教育水平、职称和营造良好的护患关系等外,护理管理者和护士应该加强血液净化专业胜任力的培养,提高护理人员的临床决策能力。

Objective:To investigate the current status of clinical decision-making ability of nurses engaged in blood purification in Shandong and to analyze its influencing factors,in order to provide a scientific basis for the improvement of the ability of the nurses.Methods:A total of 651 nurses engaged in blood purification were selected as the research objects.The general information questionnaire,nursing clinical decision-making scale and professional competency questionnaire of the nurses engaged in blood purification were used as testing and evaluation tools to conduct a survey and make single factor and Spearman correlation analysis to describe the current status of clinical decision-making ability of the nurses so as to obtain the influencing factors.Results:The total score of clinical decision-making ability of the nurses was 122 points(116,130),which was above moderate level.The total score of professional competency of the nurses was 90 points(66,115),above moderate level.The main influencing factors included gender,education level,years of working,professional title,labor relations,position and self-perception of equality of nurse-patient relationship.The total score and dimensions of the nurses′competency were positively correlated with the total score and dimensions of clinical decision-making ability(P<0.05).Conclusion:In order to improve the clinical decision-making ability of nurses engaged in blood purification,nursing managers and nurses themselves should strengthen the cultivation of professional competence so as to improve the clinical decision-making ability,besides concerning about upgrading their education level,professional title and creating a good nurse-patient relationship.
