经肛吻合器选择性痔上黏膜切除术治疗痔疮的临床效果分析

Clinical Efficacy Analysis of Selective Hemorrhoid Mucosal Resection(TST)Using Anal Stapler in the Treatment of Hemorrhoids

目的分析经肛吻合器选择性痔上黏膜切除术在痔疮治疗中的效果。方法随机选取2022年3月—2024年3月六盘水市人民医院收治的200例痔疮患者为研究对象,根据不同手术方法分为对照组与观察组,每组100例。对照组采取传统痔切除术(外剥内扎术)治疗,观察组采取选择性痔上黏膜切除术治疗。对比两组患者手术结果、手术指标以及手术前后肛肠动力学指标、疼痛评分差异和并发症。结果术后,观察组的治疗总有效率为98.00%(98/100)高于对照组的91.00%(91/100),差异有统计学意义(χ^(2)=4.714,P<0.05)。观察组的手术时间、住院时间均短于对照组,术中失血量少于对照组,差异有统计学意义(P均<0.05)。术后,观察组的肛肠动力学指标低于对照组,差异有统计学意义(P<0.05)。术后,观察组的疼痛评分低于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论临床上针对痔疮患者进行选择性痔上黏膜切除术不仅手术指标更优,其在改善痔疮患者的肛肠动力学方面效果同样显著,并降低患者术后疼痛感,临床应用价值较高。

Objective To analyze the effect of transanal stapler selective hemorrhoidectomy in the treatment of hemorrhoids.Methods A total of 200 patients with hemorrhoids admitted to Liupanshui People's Hospital from March 2022 to March 2024 were randomly selected as the study objects and divided into control group and observation group according to different surgical methods,with 100 cases in each group.The control group was treated with traditional hemorrhoids resection(external stripping and internal ligation),and the observation group was treated with selective hemorrhoids mucosal resection.The operation results,operation indexes,anorectal dynamics indexes,pain scores and complications before and after operation were compared between the two groups.Results After operation,the ttotal effective rate of treatment in the observation group was 98.00%(98/100),which was higher than 91.00%(91/100)of the control group,the difference was statistically significant(χ^(2)=4.714,P<0.05).The operation time and hospitalization time of the observation group were shorter than those of the control group,and the blood loss during operation was less than that of the control group,the differences were statistically significant(all P<0.05).After operation,the anorectal dynamics indexes of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After operation,the pain score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Clinically,selective hemorrhoid mucosal resection for hemorrhoids patients not only has better surgical indicators,but also has the same significant effect in improving the anorectal dynamics of hemorrhoids patients,and reduces the postoperative pain of patients,with high clinical application value.
