罗哌卡因腰-硬联合阻滞麻醉在老年人工髋关节置换术中的疗效及对VAS评分的影响

Effect of Ropivacaine Combined Lumbo-Epidural Block Anesthesia on Artificial Hip Replacement In Elderly Patients and Influence of VAS Score

目的研究罗哌卡因腰硬联合阻滞麻醉在老年人工髋关节置换术中的临床疗效以及对视觉模拟评分法(visual analogue scale,VAS)评分的影响效果。方法目的选取2020年2月—2023年2月期间青岛大学医疗集团西海岸第二医院收治的60例老年人工髋关节置换术治疗的患者,按照麻醉方法不同分组,对照组(n=30)实施常规全身麻醉,观察组(n=30)实施罗哌卡因腰硬联合阻滞麻醉,比较两组心率、平均动脉压、血压水平、麻醉效果、疼痛评分、Ramsay镇静评分、不良反应发生率。结果手术中,观察组平均动脉压、心率、血压水平高于对照组,差异有统计学意义(P均<0.05);苏醒时,观察组患者的疼痛评分(2.40±0.61)分、Ramsay评分(3.83±1.17)分均优于对照组的(3.56±0.81)分、(2.99±0.69)分,差异有统计学意义(t=8.298、4.373,P均<0.05);观察组患者的不良反应发生率和对照组比较,差异无统计学意义(P>0.05)。结论老年人工髋关节置换术患者通过罗哌卡因腰硬联合阻滞麻醉,可提升麻醉效果,降低疼痛。

Objective To study the clinical efficacy of ropivacaine combined spinal epidural block anesthesia in elderly patients undergoing artificial hip replacement surgery and its impact on the visual analog scale(VAS)score.Methods Sixty elderly patients who underwent artificial hip replacement surgery at the West Coast Second Hospital of Qingdao University Medical Group from February 2020 to February 2023 were objectively selected and divided into two groups by different anethesia methods.The control group(n=30)received routine general anesthesia,while the observation group(n=30)received ropivacaine combined spinal epidural block anesthesia.The heart rate,mean arterial pressure,blood pressure,anesthetic effec,pain scores and Ramsay scores,incidence of adverse reactions.Results During surgery,the average arterial pressure,heart rate,and blood pressure levels of the observation group were higher than those of the control group,the difference was statistically significant(all P<0.05);At the time of awakening,the pain score 2.40±0.61 and Ramsay score 3.83±1.17 of the observation group were better than 3.56±0.81 and 2.99±0.69 of the control group,the difference was statistically significant(t=8.298,4.373,both P<0.05);The incidence of adverse reactions in the observation group was not significant compared to the control group,and the difference was not statistically significant(P>0.05).Conclusion Elderly patients undergoing artificial hip replacement can improve anesthesia efficacy and reduce pain through ropivacaine combined spinal epidural block anesthesia.
