三维后装精准放疗联合体外调强外照射治疗宫颈癌的疗效及安全性

Effect and safety of three-dimensional after loading precision radiotherapy combined with external intensity-modulated radiation in the treatment of cervical cancer

目的探究宫颈癌采用三维后装精准放疗联合体外调强外照射治疗的疗效及安全性。方法对2018年4月—2021年4月期本院收治84例宫颈癌患者资料进行回顾性分析,患者均接受三维后装精准放疗,将加用体外调强外照射治疗与常规放疗治疗者分别纳入研究组44例与对照组40例。比较两组患者治疗疗效,治疗前后肿瘤标志物,治疗前后生存质量,药物毒副作用,1年无复发生存率。结果研究组患者缓解率均高于对照组,差异有统计学意义(95.45%vs 77.50%,P<0.05);两组患者治疗后细胞角蛋白19片段抗原21-1(CYFRA21-1)、鳞状细胞癌抗原(SCC)以及恶性肿瘤特异生长因子(TSGF)等肿瘤标志物水平均明显下降(P<0.05),治疗后研究组患者CYFRA21-1、SCC、TSGF水平低于对照组(P<0.05);两组患者治疗后Karnofsky评分均明显上升(P<0.05),治疗后研究组患者Karnofsky评分高于对照组(P<0.05);研究组患者治疗后放射性膀胱炎、消化道反应、放射性直肠炎、肝功能损伤以及骨髓抑制等药物毒副反应发生率均显著低于对照组(P<0.05);研究组与对照组患者1年无复发生存率分别为95.5%与75.0%,差异有统计学意义(χ^(2)=6.797,P<0.001)。结论宫颈癌采用三维后装精准放疗联合体外调强外照射治疗疗效及安全性优异。

Objective To explore the effect and safety of three-dimensional after loading precision radiotherapy combined with external intensity-modulated radiation in the treatment of cervical cancer.Methods A retrospective analysis was performed on the data of 84 patients with cervical cancer admitted to the hospital from April 2018 to April 2021.All underwent three-dimensional after loading precision radiotherapy.After that,those who underwent external intensity-modulated radiation and routine radiotherapy were included into study group(n=44)and control group(n=40),respectively.The curative effect,tumor markers and quality of life before and after treatment,drug side effects and 1-year recurrence-free survival rate were compared between the two groups.Results The remission rate in study group was higher than that in control group(95.45%vs 77.50%,P<0.05).After treatment,levels of tumor markers[cytokeratin 19 fragment antigen 21-1(CYFRA21-1),squamous cell carcinoma(SCC)antigen,tumor-specific growth factor(TSGF)]in both groups were significantly decreased(P<0.05),which were lower in study group than control group(P<0.05).After treatment,Karnofsky scores in both groups were significantly increased(P<0.05),which were higher in study group than control group(P<0.05).The incidence of drug side effects(radiation cystitis,gastrointestinal reactions,radiation proctitis,liver damage,bone marrow suppression)in study group was significantly lower than that in control group(P<0.05).The differences in 1-year recurrence-free survival rate between study group and control group were statistically significant(95.5%vs 75.0%)(χ^(2)=6.797,P<0.001).Conclusion Effect and safety of three-dimensional after loading precision radiotherapy combined with external intensity-modulated radiation are excellent in the treatment of cervical cancer.
