经脐单孔腹腔镜胆囊切除术治疗胆囊息肉的临床效果

Clinical Efficacy of Single Port Laparoscopic Cholecystectomy Via Umbilical Cord for the Treatment of Gallbladder Polyps

目的研究经脐单孔腹腔镜胆囊切除术治疗胆囊息肉的临床效果。方法以酒泉市人民医院2022年11月至2023年7月收治的102例胆囊息肉患者为研究对象,按照随机数字表法分为对照组和观察组,各51例。对照组采用传统三孔腹腔镜胆囊切除术治疗,观察组采用经脐单孔腹腔镜胆囊切除术治疗。对比两组两组疗效、术后并发症、术后疼痛、手术相关指标、炎症因子水平。结果两组治疗总有效率、术后并发症发生率对比,P>0.05。治疗后,观察组VAS评分较对照组更低,P<0.05。观察组术中出血量较对照组更少,肠鸣音恢复时间、住院时间均较对照组更短,腹壁美容度评分较对照组更高,P<0.05。治疗后,两组CRP、IL-8、6等炎症因子水平均有所上升,但观察组较对照组低,P<0.05。结论经脐单孔腹腔镜胆囊切除术治疗胆囊息肉的临床效果显著,且并发症发生率较低,同时还具有术后患者恢复更快、疼痛更轻、炎症反应更小等优势。

Objective:To study the clinical effect of transumbilical single-port laparoscopic cholecystectomy in the treatment of gallbladder polyps.Methods:102 patients with gallbladder polyps in Jiuquan People's Hospital from November 2022 to July 2023 were taken as the research object,and were randomly divided into control group and observation group,with 51 cases in each group.The control group was treated with traditional three-hole laparoscopic cholecystectomy,while the observation group was treated with transumbilical single-hole laparoscopic cholecystectomy.The curative effect、postoperative complications、postoperative pain、surgical related indexes and inflammatory factors were compared between the two groups.Results:The total effective rate and postoperative complications of the two groups were compared,P>0.05.After treatment,the VAS score of the observation group was lower than that of the control group,P<0.05.The amount of bleeding in the observation group was less than that in the control group,and the recovery time and hospitalization time of bowel sounds were shorter than those in the control group,and the abdominal wall beauty score was higher than that in the control group,P<0.05.After treatment,the levels of inflammatory factors such as CRP、IL-8 and IL-6 in both groups increased,but the levels in the observation group were lower than those in the control group,P<0.05.Conclusion:Transumbilical single-port laparoscopic cholecystectomy is effective in the treatment of gallbladder polyps,and the incidence of complications is low.At the same time,it has the advantages of faster recovery,less pain and less inflammatory reaction.
