摘要
目的分析氧化锆与钴铬合金修复材料对口腔修复患者修复成功率及龈沟液(GCF)中炎症因子水平的影响。方法将洛阳市第三人民2019年9月至2022年9月接受口腔修复的86例患者,根据修复方案分为A组(接受氧化锆修复体治疗)、B组(接受钴铬合金修复治疗),两组各43例。比较两组修复成功率、牙周指标[牙龈出血指数(BI)、牙周探诊深度(PD)、菌斑指数(PLI)、GCF量]、GCF炎症因子[白介素-8(IL-8)、白介素-6(IL-6)、基质金属蛋白酶-2(MMP-2)]水平、骨变化相关因子[肿瘤坏死因子相关弱凋亡诱导因子(TWEAK)、破骨细胞核因子κB受体活化因子配体(RANKL)、RANKL/破骨细胞抑制因子(OPG)]水平及牙齿功能。结果A组修复成功率高于B组(P<0.05)。治疗6个月后B组BI、PD升高(P<0.05),A组BI、PD低于B组(P<0.05)。治疗后两组IL-8、IL-6、MMP-2水平均升高,且B组高于A组(P<0.05)。治疗后两组TWEAK、RANKL、RANKL/OPG均升高,且B组高于A组(P<0.05)。与B组相比,治疗后A组咀嚼功能、舒适度、牙齿美观度、语言功能评分升高幅度较明显(P<0.05)。结论氧化锆修复体对口腔修复患者牙周组织刺激较钴铬合金修复体小,且GCF中炎症因子水平较低,对牙槽骨骨质吸收程度影响较轻,牙齿功能更强,修复成功率更高。
Objective To analyze the effects of zirconia and cobalt-chromium alloy repair materials on the success rate of repair and the level of inflammatory factors in gingival crevicular fluid(GCF)in patients with oral repair.Methods A total of 86 patients who received oral prosthesis from September 2019 to September 2022 in Luoyang Third People’s Hospital were divided into group A(received zirconia prosthesis treatment)and group B(received cobalt-chromium alloy prosthesis treatment)according to prosthetic schemes,with 43 cases in each group.The success rate of restoration,periodontal indicators[gingival bleeding index(BI),periodontal probing depth(PD),plaque index(PLI),GCF volume],GCF inflammatory factors[interleukin-8(IL-8),interleukin-6(IL-6),matrix metalloproteinase-2(MMP-2)],bone change-related factors[tumor necrosis factor-related weak apoptosis-inducing factor(TWEAK),receptor activator of nuclear factor-κB ligand(RANKL),RANKL/osteoprotegerin(OPG)],and dental function were compared between the two groups.Results The success rate of repair in group A was higher than that in group B(P<0.05).After 6 months of treatment,BI and PD increased in group B(P<0.05),while BI and PD were lower in group A than in group B(P<0.05).After treatment,the levels of IL-8,IL-6,and MMP-2 in both groups increased,and the level in group B was higher than that in group A(P<0.05).After treatment,TWEAK,RANKL,and RANKL/OPG levels increased in both groups,and group B was higher than group A(P<0.05).Compared with group B,the scores of masticatory function,comfort,dental aesthetics,and language function in group A increased significantly after treatment(P<0.05).Conclusion Compared with cobalt-chromium alloy prosthesis,zirconia prosthesis has less stimulation on periodontal tissue of patients with oral prosthesis,and the level of inflammatory factors in GCF is lower,which has less influence on the degree of alveolar bone absorption,stronger tooth function and higher success rate of prosthesis.
