舒血宁注射液联合塞来昔布对椎动脉型颈椎病患者血流动力学及血清ET-1、NO水平的影响

摘要 目的观察舒血宁注射液联合塞来昔布治疗椎动脉型颈椎病(CSA)的临床效果,探究其对血流动力学、血管内皮功能的影响。方法选取2019年6月至2022年6月河南科技大学第一附属医院收治的90例CSA患者为研究对象,其中45例接受塞来昔布治疗(设为对照组),45例接受舒血宁注射液联合塞来昔布治疗(设为研究组)。两组治疗10~12 d后对比治疗效果、不良反应。比较两组治疗前后颈椎活动度(ROM)、颈性眩晕症状与功能评估(ESCV)、颈椎功能障碍指数(NDI)、血流动力学[椎动脉-基底动脉平均血流速度(TMFV)、收缩期峰值血流速度(PSV)、血管搏动指数(PI)、阻力指数(RI)]、血管内皮功能[内皮素-1(ET-1)、一氧化氮(NO)、血栓素A2(TXA2)]。结果与对照组比较,研究组总有效率升高(P<0.05);与对照组比较,研究组治疗后ROM、NDI评分降低,ESCV评分升高(P<0.05);与对照组比较,研究组治疗后TMFV、PSV升高,PI、RI降低(P<0.05);与对照组比较,研究组治疗后血清ET-1、TXA2水平降低,NO水平升高(P<0.05);两组均未出现严重不良反应。结论舒血宁注射液联合塞来昔布治疗CSA的效果确切且安全可靠,可改善颈椎活动度、颈椎功能、血流动力学,促进血管内皮功能恢复。 Objective To observe the clinical efficacy of Shuxuening injection combined with celeoxib in the treatment of vertebral artery type cervical spondylosis of vertebral artery type(CSA),and to explore its influence on hemodynamics and vascular endothelial function.Methods A total of 90 patients with CSA admitted to the First Affiliated Hospital of Henan University of Science and Technology from June 2019 to June 2022 were selected as the study objects,45 of whom were treated with celecoxib(set as control group)and 45 with Shuxuening injection combined with celecoxib(set as study group).After 10-12 days of treatment,the therapeutic effect and adverse reactions were compared between the two groups.Range of motion(ROM),evaluation scale for cervical vertigo(ESCV),neck disability index(NDI),hemodynamics[time-mean flow velocity(TMFV),peak systolic blood flow velocity(PSV),pulse index(PI),resistance index(RI)]and vascular endothelial function[endothelin-1(ET-1),nitric oxide(NO),thromboxane A2(TXA2)]were compared between the two groups before and after treatment.Results Compared with the control group,the total effective rate of the study group was significantly increased(P<0.05).Compared with the control group,the ROM score and NDI score of the study group were decreased after treatment,and the ESCV score was increased(P<0.05).Compared with the control group,TMFV and PSV were increased and PI and RI were decreased in the study group after treatment(P<0.05).Compared with the control group,the serum levels of ET-1 and TXA2 in the study group were decreased after treatment,while the level of NO was increased(P<0.05).There were no serious adverse reactions in both groups.Conclusion Shuxuening injection combined with celecoxib is effective,safe and reliable in the treatment of CSA,which can improve cervical motion,cervical function and hemodynamics,promote the recovery of vascular endothelial function.
