中青年上消化道出血病人创伤后成长现状及影响因素分析

Status quo and influencing factors of post-traumatic growth in young and middle-aged patients with upper gastrointestinal bleeding

目的:调查中青年上消化道出血病人创伤后成长现状并分析其影响因素,为临床制订干预方案提供参考依据。方法:采用一般资料调查表、创伤后成长量表对2023年1月—10月银川市某三级甲等医院消化内科收治的145例中青年上消化道出血病人进行调查,采用多重线性回归方法分析其创伤后成长的影响因素。结果:中青年上消化道出血病人创伤后成长得分为(54.57±16.33)分;多重线性回归分析显示,有焦虑症或抑郁症、婚姻状态、年龄和患病时间是影响其创伤后成长的独立因素(调整后R^(2)=0.313,F=6.969,P<0.001)。结论:中青年上消化道出血病人创伤后成长处于中等水平,临床实践中应重点关注存在负性心理、未婚、30~40岁患病不足1年的病人,针对亚人群具体特点制定相应护理策略,以确保病人心理健康,促进疾病转归与康复。

Objective:To investigate the status quo of post-traumatic growth of middle-aged and young patients with upper gastrointestinal hemorrhage and analyze its influencing factors,and provide reference for clinical intervention.Methods:A total of 145 young and middle-aged patients with upper gastrointestinal bleeding admitted to the gastroenterology department of Third grade A hospital in Yinchuan city from January to October 2023 were investigated by using the general data questionnaire and post-traumatic growth scale.Multiple linear regression method was used to analyze the influencing factors of post-traumatic growth.Results:The total score of post-traumatic growth in young and middle-aged patients with upper gastrointestinal bleeding was 54.57±16.33.The results of multiple linear regression analysis showed that the anxiety or depression,marital status,age and course of disease were the independent factors affecting the post-traumatic growth of patients with upper gastrointestinal bleeding(adjusted R~2=0.313,F=6.969,P<0.001).Conclusions:The post-traumatic growth of middle-aged and young patients with upper gastrointestinal bleeding is at a moderate level.In clinical practice,we should focus on patients with negative psychology,unmarried,30-40 years old and patients with less than 1 year of illness,and develop corresponding nursing strategies according to the specific characteristics of the subpopulation,so as to ensure the mental health of patients and promote the outcome and rehabilitation of the disease.
