茵陈蒿汤加味药浴对新生儿黄疸的临床疗效及光疗终点事件的影响

Clinical Effect of Yinchenhao Decoction on Neonatal Jaundice and the Influence of Phototherapy Endpoint Events

摘要 目的探讨茵陈蒿汤加味药浴对新生儿黄疸的临床疗效及光疗终点事件的影响。方法选取2020年2月至2022年3月大连市妇女儿童医疗中心门诊收治的新生儿黄疸患儿90例为研究对象,按照随机数字表法分为对照组与茵陈蒿汤加味药浴组(以下简称药浴组)。对照组采用常规疗法,患儿口服益生菌,晒太阳,并采用一般水疗,药浴组在对照组基础上予以中药浴。比较两组患儿排便情况(包括排便次数、胎便转黄时间)、睡眠时间、临床疗效、血清胆红素变化情况及累计未做蓝光率。结果治疗后,与对照组比较,药浴组每日排便次数更多,胎便转黄时间较短,每日睡眠时间较长,差异有统计学意义(P<0.05);药浴组治疗有效率高于对照组(P<0.05);治疗第3天、第5天、第7天,药浴组血清胆红素水平低于对照组(P<0.05);治疗后第5天、10天、14天药浴组累计未做蓝光率高于对照组(P<0.05)。结论茵陈蒿汤加味药浴能够有效缓解新生儿黄疸患儿症状,降低血清胆红素水平,使达到光疗指征的患儿减少。 Objective To explore the clinical effect of Yinchenhao Decoction on neonatal jaundice and the influence of phototherapy endpoint events.Methods A total of 90 children with neonatal jaundice treated in the outpatient department of Dalian Women and Children's Medical Center from February 2020 to March 2022 were selected as the study objects,and were divided into control group and Yinchenhao Decoction supplemented medicinal bath group(hereinafter referred to as medicinal bath group)according to random number table method.The control group received conventional treatment,the children took oral probiotics,basked in the sun,and received general spa treatment,and the medicinal bath group added Chinese medicinal bath on the basis of the control group.The defecation situation(including the frequency of defecation,the time of fetal stool turning yellow),sleep time,clinical efficacy,the change of serum bilirubin and the accumulated blue light rate were compared between the two groups.Results After treatment,compared with the control group,the daily defecation times in the medicated bath group were more,the yellow turning time of fetal stool was shorter,and the daily sleep time was longer,the difference was statistically significant(P<0.05).The effective rate of medicine bath group was higher than that of control group(P<0.05).On the 3rd,5th and 7th day of treatment,the serum bilirubin level in the medicated bath group was lower than that in the control group(P<0.05);On the 5th,10th and 14th day after treatment,the accumulated blue light rate of the drug bath group was higher than that of the control group(P<0.05).Conclusion Yinchenhao Tang modified medicinal bath can effectively alleviate the symptoms of neonatal jaundice,reduce serum bilirubin,and reduce the number of children who meet the indications for phototherapy.
