砭石热熨加按揉经穴治疗项背肌筋膜炎的随机对照试验

A randomized controlled trial of stone-needle used in hot compress combined with massage on points along channels for nuchal dorsal myofascitis

目的探讨砭石热熨加按揉经穴对项背肌筋膜炎患者的临床疗效。方法采用多中心、随机对照临床试验方法,将2022―2023年在不同地区的3家三甲中医医院就诊的项背肌筋膜炎患者112例随机分为试验组(56例)和对照组(56例),试验组给予砭石热熨加按揉经穴治疗,对照组给予氟比洛芬凝胶贴膏治疗,疗程为2周。对比干预前后2组患者整体疼痛量表(GPS)评分、关节活动度的变化。结果治疗2周后,试验组患者的GPS整体疼痛评分明显低于对照组,差异有统计学意义(P<0.05)。试验组患者颈后伸、肩外展和肩水平伸展活动度改善程度明显优于对照组,差异有统计学意义(P<0.05)。结论砭石热熨加按揉经穴可降低项背肌筋膜炎患者疼痛程度,改善颈、肩关节活动度。

Objective To investigate the clinical efficacy of stone-needle used in hot compress combined with massage on the points along the channels for patients with nuchal dorsal myofascitis.Methods A multicenter,randomized controlled clinical trial was adopted,and a total of 112 patients with nuchal dorsal myofascitis who were treated in three tertiary TCM hospitals in three different regions from 2022 to 2023 were randomly divided into experimental group(56 cases)and control group(56 cases).The patients in the experimental group were treated with stone-needle for hot compress combined with massage on the channel points,whereas those in the control group were treated with flurbiprofen gel plaster,with the course of treatment of 2 weeks.The changes of global pain scale(GPS)score and the range of motion between the two groups before and after treatment were compared.Results After 2 weeks of treatment,the GPS score of the experimental group was significantly lowered than that of the control group,with statistically significant difference(P<0.05).The improvement in the range of motion of neck posterior extension,shoulder abduction,and shoulder horizontal extension in the experimental group was significantly better than that in the control group,with statistically significant difference(P<0.05).Conclusion Stone-needle for hot compress combined with massage on the channel points can reduce pain and improve the range of motion of neck and shoulder joints in patients with fasciitis of nape muscle.
