摘要
目的分析甲襞微循环与皮肌炎患者疾病活动度的相关性。方法收集20例皮肌炎患者(皮肌炎组)和20例健康体检者(对照组)的临床资料,采用甲襞视频毛细血管镜检测皮肌炎组甲襞微循环,肌炎活动视觉模拟评估工具评估疾病活动度,Pearson相关性分析甲襞微循环与疾病活动度之间相关性。结果与对照组比较,皮肌炎组毛细血管袢数量较少,毛细血管袢顶直径较大,微出血比例、ESR、CRP较高(P<0.05)。皮肌炎组总疾病活动度为(4.48±1.18)分,肌肉疾病活动度为(4.62±1.24)分,肺脏疾病活动度为(2.37±0.90)分。毛细血管袢数量与肌肉疾病活动度和肺脏疾病活动度呈负相关(r=-0.907和-0.500,P<0.05),毛细血管袢顶直径与总疾病活动度、肌肉疾病活动度和肺脏疾病活动度呈正相关(r=0.462、0.644和0.523,P<0.05)。结论甲襞微循环与皮肌炎患者疾病活动度密切相关。
Objective To analyze the correlation between nailfold microcirculation and disease activity in the patients with dermatomyositis.Methods The clinical data of 20 patients with dermatomyositis(group A)and 20 healthy people(group B)were collected.The nailfold microcirculation in group A was detected by nailfold video capillary scope and disease activity was accessed by visual simulation assessment tool for myositis activity.The correlation between nail fold microcirculation and disease activity was evaluated by Pearson correlation analysis.Results Compared to group B,the number of capillary loops was less,the top diameter of capillary loops was larger,and the proportions of microbleeds,ESR and CRP were higher in group A(P<0.05).The total disease activity was(4.48±1.18)points,the muscle disease activity was(4.62±1.24)points,and the lung disease activity was(2.37±0.90)points in group A.The number of capillary loops was negatively correlated with muscle disease activity and lung disease activity(r=-0.907 and-0.500,P<0.05).The top diameter of capillary loops was positively correlated with total disease activity,muscle disease activity and lung disease activity(r=0.462,0.644 and 0.523,P<0.05).Conclusion Nailfold microcirculation is closely correlated with disease activity in the patients with dermatomyositis.
