多层螺旋CT双低剂量和常规剂量在结肠癌肿瘤侵犯、肠系膜血管分布中准确率的效果对比

Comparison of Accuracy of Double Low-dose and Conventional Multi-slice Spiral CT in Colorectal Cancer Invasionand Mesenteric Vascular Distribution

目的:探究多层螺旋电子计算机断层扫描(MSCT)双低剂量和常规剂量在结肠癌肿瘤侵犯、肠系膜血管分布中准确率的效果对比。方法:选取2021年6月—2022年6月厦门市中医院收治的69例结肠癌患者作为研究对象,按照随机数表法分为常规组(n=35,常规剂量MSCT)与双低组(n=34,双低剂量MSCT)。对比两组图像质量评价[噪声、CT值,计算信噪比(SNR)与对比信噪比(CNR)以及医师主观评价分数]、辐射剂量[剂量长度乘积(DLP)、容积CT剂量指数(CTDIvol)、有效剂量(ED)]及肿瘤侵犯、肠系膜血管分布准确率。结果:双低组噪声及CT值显著高于常规组,但SNR及CNR值显著低于常规组,差异有统计学意义(P<0.05);两组图片主观评分比较,差异无统计学意义(P>0.05);双低组DLP、CTDIvol、ED均显著低于常规组,差异有统计学意义(P<0.05);常规组与双低组检测直肠癌肿瘤侵犯及肠系膜血管分布准确率,差异无统计学意义(P>0.05)。结论:双低剂量MSCT可显著减少辐射暴露,可准确显示结肠癌肿瘤侵犯及肠系膜血管分布情况,满足影像学诊断要求,且安全性更高,有较好的应用价值。

Objective:To compare the accuracy of double low dose and conventional dose of MSCT in colon cancer invasion and mesenteric vascular distribution.Method:Sixty-nine colon cancer patients admitted to the hospital from June 2021 to June 2022 were divided into the conventional group(n=35,conventional-dose MSCT)and the double-low group(n=34,double-low-dose MSCT)according to the randomized numerical table method.The image quality evaluations[noise,CT value,computed signal-to-noise ratio(SNR)and contrast signal-to-noise ratio(CNR)and subjective evaluation scores of the physicians],radiation dose[dose-length product(DLP),volumetric CT dose index(CTDIvol),effective dose(ED)]and tumor invasion,mesenteric vascular distribution accuracy were compared between the group.Result:The noise and CT values of the double-low group were significantly higher than those of the conventional group(P<0.05).However,the comparison of SNR and CNR values were significantly lower than those of the conventional group(P<0.05).There was no significant difference in the subjective scores of the pictures of the two groups(P>0.05).the DLP,CTDIvol,and ED of the double-low group were significantly lower than those of the conventional group(P<0.05).there was no significant difference in the accuracy rate of detecting tumor invasion and mesenteric vascular distribution between the conventional group and the double-low group in rectal cancer.There is no significant difference in accuracy between conventional group and double-low group(P>0.05).Conclusion:Double-low-dose MSCT can significantly reduce radiation exposure,accurately display colon cancer tumor invasion and mesenteric vascular distribution,meet the requirements of diagnostic imaging,and be safer,so it has a better value of application.
