芪参养心汤治疗心肾阳虚证慢性心力衰竭的临床效果

Clinical Efficacy of Qishen Yangxin Decoction in the Treatment of Chronic Heart Failure with Heart-kidney Yang Deficiency Syndrome

目的:观察芪参养心汤治疗心肾阳虚证慢性心力衰竭的临床效果。方法:选取2022年1月—2023年1月湖南中医药大学第二附属医院住院部收治的60例心肾阳虚证CHF患者。根据随机数表法将其分为对照组(n=30)和研究组(n=30)。对照组给予常规西医治疗,研究组在对照组基础上给予芪参养心汤治疗。比较两组治疗前后中医症候积分、生活质量、相关指标,临床疗效,再住院情况及不良反应。结果:治疗后,研究组中医症候积分低于对照组,健康调查量表(SF-36)评分高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组B型利钠肽(BNP)、左心室舒张末期内径(LVEDD)均低于对照组,6分钟步行试验(6MWT)、左心室射血分数(LVEF)均高于对照组,差异有统计学意义(P<0.05)。研究组总有效率高于对照组,差异有统计学意义(P<0.05)。研究组再住院率及不良反应发生率均低于对照组,差异有统计学意义(P<0.05)。结论:芪参养心汤治疗心肾阳虚证CHF,能有效减轻患者临床症状,改善心室重构,提高心脏泵血能力,改善心脏功能及运动耐量,提高生活质量及临床疗效,改善患者远期预后,且具有较高安全性。

Objective:To observe the clinical efficacy of Qishen Yangxin Decoction in the treatment of chronic heart failure with heart-kidney Yang deficiency syndrome.Method:A total of 60 CHF patients with heart-kidney Yang deficiency syndrome were selected from the Inpatient Department of the Second Affiliated Hospital of Hunan University of Chinese Medicine from January 2022 to January 2023.They were divided into control group(n=30)and study group(n=30)according to random number table method.The control group was given conventional western medicine treatment,and the study group was given Qishen Yangxin Decoction on the basis of the control group.The scores of traditional Chinese medicine symptoms,quality of life,related indexes before and after treatment,clinical efficacy,re-hospitalization condition and adverse reactions were compared between the two groups.Result:After treatment,the Chinese medicine syndrome score of the study group was lower than that of the control group,and the health survey scale(SF-36)score was higher than that of the control group,the differences were statistically significant(P<0.05).After treatment,B type natriuretic peptide(BNP)and left ventricular end diastolic diameter(LVEDD)in the study group were lower than those in the control group,while the 6-minute walking test(6MWT)and left ventricular ejection fraction(LVEF)were higher than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The rate of re-hospitalization and the incidence of adverse reactions in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Qishen Yangxin Decoction in the treatment of heartkidney Yang deficiency syndrome CHF can effectively reduce clinical symptoms of patients,improve ventricular remodeling,improve heart pumping ability,improve heart function and exercise tolerance,improve quality of life and clinical efficacy,improve long-term prognosis of patients,and have high safety.
